Overview

As Local Coordinator and as part of the Blended Care Team, you will assist the care management team in developing social activities and interest groups for residents at a physical centre, under the Health District @ Queenstown. This effort is in part to support social-health integration and enhance care delivery in the community.

These activities include working with residents and community partners to co-create, plan and conduct ground-up activities (e.g. exercise groups, hawker centre tours, etc.), encourage residents to participate in suitable programmes, within and beyond the physical space and oversee the general operations at the centre.

You will be involved in facilitating the development and implementation of Health District Queenstown Lighthouse Projects involving National University Health System (NUHS) and various community partners in Queenstown to support social-health integration and enhance care delivery in the community.

Job Responsibilities

You will be responsible for the following:

1. Co-create and organise activities and interest groups at the physical space

· Engage residents and volunteers to co-create, plan, and conduct activities and interest groups e.g. hawker centre tours to share on healthier eating habits.

· Encourage and support residents to self-run activities and interest groups

· Partner with local community leader(s) and stakeholders to organise programmes and services

· Organise regular resident engagement sessions to gather feedback on activities and services provided by the Blended Care Team

· Bachelor Degree in the field of Social Sciences (e.g. social work, counselling, psychology), with prior experience in related areas such as social care services, aged care services and/or community engagement.

The incumbent will be responsible for supporting the core functions of the MSc degree programmes which includes student life, experiential learning, and administration matters.

Corporate Relationship Development

• Establish corporate contacts through a systematic outreach process, follow up on new business projects generation opportunities, maintain working relationships and set up meetings
• Pitch initiatives to corporates, specifically in positioning and securing business projects for our respective MSc programmes
• Scope the business projects with the company, establish an understanding of the objectives, guidelines for workload, deliverables, resources and financial support
• Solicit for feedback on our students’ performances to make recommendations for improvements on projects and associated activities
• Feature success stories prominently in our marketing collaterals and channels such as brochures, websites and e-blasts to encourage more participation from different corporate partners

Administration of Management Practicum

• Collate a list of business projects, disseminate to our respective MSc students for enrolment, and where needed, conduct interviews for the allocation of projects

• Coordinate coaching workshops/classes to prepare students for their undertaking of business projects
• Work with Heads of Departments in appointing project supervisors, including briefing faculty advisors on the role, responsibilities and scope of projects

As a Senior Executive/Assistant Manager, you are responsible to support the day-to-day operations of the Care and Call Centre (CareHub) to meet CareHub’s service standards, key performance indicators (KPIs) and deliver a positive customer experience for residents and community partners who reach in to CareHub for support to age well in the community.

Key accountabilities but not limited to:

1. Support the day-to-day operations of the CareHub including being the point of escalation when required, in line with CareHub’s service standards, key performance indicators (KPIs), developmental plans and organisation’s policies
2. Support in the implementation of CareHub’s functions and IT systems through monitoring and tracking of timelines, translating plans and requirements into actionable workflows, development/review of SOPs to working collaboratively with stakeholders, vendors and users to propose go-forward solutions to meet implementation timeline
3. Facilitate collaborative conversations and change management with presentation slides internally and with stakeholders to further develop CareHub’s functions and facilitate adoption of new/improved workflows and SOPs
4. Support in the continuous development and implementation of projects and quality initiatives to enhance operational effectiveness and efficiency with a focus on providing a positive customer experience in line with CareHub’s standards and KPIs.

6. Ensure that Call Centre Officers are competent and trained in the most efficient and timely manner supported by a relevant knowledge management system
7. Data visualization and reporting of CareHub’s KPIs and standards to stakeholders and senior management in a concise and timely manner
8. Utilise data driven approach to identify opportunities and forecast growth areas / trends to further develop CareHub
9. Keep abreast of the current issues impacting the healthcare industry and conduct periodic scanning for new ways and modes which CareHub could better support our residents and partners

• Good bachelor’s degree in any discipline.

Day to Day Activities

• Collaborate with the Senior Finance Manager to oversee the company’s financial well-being and support key compliance areas, including statutory audits, corporate tax, GST, withholding tax, and transfer pricing.
• Provide guidance to Finance Executives and Associates by coordinating tasks and addressing inquiries.
• Ensure timely and accurate month-end and year-end closings, including preparing accounting ledgers, journals, schedules, and financial statements for internal and external requirements.
