• Assist in initial assessment of client/caregiver needs with basic case management under the direction of a case manager/ social worker
• Ensure client and caregiver’s ability to cope by helping to streamline and coordinate the range of community services available
• Assist client/caregiver with appropriate information and referral assistance
• Conduct joint home visits with a case manager/ social worker or the multidisciplinary team where necessary
• Participate in case discussion with the clinical team
• Consolidate, prepare, track relevant monthly reports and statistics

Job Requirement:

About the Role:

As an experienced educator, you will play a critical role in helping students excel in English, Math, and Science, while also supporting students who require additional help with Chinese language proficiency.
