Key Summary
The Nurse/ Senior Nurse Clinician/ Advanced Practice Nurse (APN) is responsible for monitoring and evaluating the effectiveness, safety and efficiency of community nursing practices within the Foundation’s range of clinical services.
Key Responsibilities
- Develop and implement evidence-based clinical practices, guidelines, policies and procedures, including defining nursing competencies and care standards through working with teams
- Collaborate with cross-functional teams in clinical service strategic planning work and identify areas of opportunities and growth for clinical programs and service
- Network and conduct market research and landscape studies to stay abreast of emerging trends / technologies / innovations to support the development of new nursing care models and implements strategies to augment care delivery with seamless integration of programs and services among clinical teams within the Foundation. Cross-functional teamwork and inter-agency collaborations are paramount for success
- Well-versed in community and gerontological nursing practices, coupled with a good understanding of the health profile and population needs of older people in the community, to ensure service alignment with the Foundation’s philosophy of care
- By applying evidence-based community gerontological nursing practices, he/she shall effectively manage clients with complex care needs through direct care
- Together with the Senior Team to manage a team of community nurses across the Foundation and be responsible for provision of clinical supervision, coaching and mentoring, evaluation of care standards and quality, development and assessment of nursing competency, including identification and implementation of training programs to support the on-going professional development and retention of nursing staff
- With collaboration with cross-functional teams, to develop and implement audit protocols and procedures to evaluate compliance, identify and address gaps in clinical care and service delivery as well as be involved in risk management and clinical excellence initiatives
- Lead quality improvement initiatives and conduct/support clinical research projects and advocacy works to advance knowledge and improve clinical outcomes
- Participate in collaborative projects with internal and external stakeholders to advocate for quality of care to community dwelling older adults
- APN will be more clinical focus and more details will be shared during interview.
Key Requirements
- Minimum Masters/Bachelor in Nursing with a Post-Graduate/Specialist Diploma in Gerontological Nursing/Gerontology
- Additional professional qualification/certification in public health, public policy, business management or equivalent will be advantageous
- Registered with the Singapore Nursing Board
- At least 12 – 15 years of nursing experience, of which 5 – 8 years are in clinical supervision, nursing training and education
- Hands-on experience with older adults in community-based care settings is essential
- Prior appointment in management roles and work experience in case management or care coordination will be advantageous
- Extensive clinical knowledge and skill
- Competent communication, leadership and negotiation skills
- Resilient and resourceful with a creative flair and aptitude to excel in a dynamic environment with evolving requirements and different team dynamics.
Salary range is indicative and will be commensurate with qualification and experience.
