Job Description

• Conduct comprehensive assessments of residents' health conditions and develop individualized care plans.
• Monitor and evaluate the effectiveness of care plans and adjust as needed to ensure optimal outcomes.
• Administer medications and treatments in accordance with physician orders and nursing standards.
• Provide specialized care for residents with chronic conditions, dementia, or palliative care needs.
• Act as a clinical resource for nursing staff, providing guidance and support to improve skills and care quality.
• Oversee the implementation of evidence-based practices and maintain high clinical standards.
• Participate in quality improvement initiatives and audits to enhance care delivery.

Requirements

• Min Diploma / Degree in nursing
• Min 5 years of experience in similar settings
