Job Summary:
The Counsellor will provide comprehensive psychosocial support to individuals and families, relationship and emotional support, end-of-life care, and the implementation and evaluation of relevant programs. The ideal candidate will possess strong interpersonal skills, a deep understanding of psychosocial dynamics, and the ability to create a supportive environment for residents.
Key Responsibilities:
Psychosocial Support:
- Provide individual and group counselling sessions to residents, focusing on emotional, psychological, and social well-being.
- Offer relationship support by helping clients navigate interpersonal issues and fostering healthy communication.
End-of-Life Support:
- Provide compassionate support to residents and families facing end-of-life issues, ensuring dignity and respect throughout the process.
- Collaborate with healthcare providers to develop and implement care plans that address both emotional and practical needs during end-of-life transitions.
- Conduct grief and bereavement counselling for families coping with loss.
Program Implementation:
- Assist in the development, implementation, and management of psychosocial programs aimed at enhancing residents' quality of life.
- Work collaboratively with the MSW and other team members to identify community needs and tailor programs accordingly.
- Facilitate workshops and training sessions on relevant psychosocial topics for residents, families, staff and community members.
Program Evaluation:
- Monitor and evaluate the effectiveness of psychosocial programs through data collection, feedback, and outcome assessments.
- Prepare and present reports on program performance, highlighting successes, challenges, and areas for improvement.
- Utilize evaluation findings to inform future program development and enhance service delivery.
Collaboration and Networking:
- Establish and maintain relationships with community organizations, healthcare providers, and other stakeholders to support residents' needs.
- Participate in multidisciplinary team meetings to ensure a holistic approach to resident care.
Documentation and Compliance:
- Maintain accurate and confidential records of resident interactions, assessments, and progress notes in accordance with organizational policies and legal requirements.
- Ensure compliance with ethical standards and best practices in counselling and social work.
Professional Development:
- Engage in continuous professional development through training, workshops, and relevant certifications.
- Stay informed about current trends and best practices in psychosocial support and counselling.
Qualifications:
- Degree in Counselling, Psychology, Social Work or related field.
- Relevant licensure or certification in counselling or social work (preferred).
- Proven experience in providing psychosocial support and counselling.
- Strong understanding of end-of-life issues and grief counselling.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Commitment to client confidentiality and ethical practice.
