Duties & Responsibilities

You shall work under the direction of Programme Manager(s). Your main duties and responsibilities include the following:

FOCY team

• Conduct comprehensive child and family assessments, identifying strengths, challenges, and needs of families impacted by parental incarceration.
• Provide individual and family counselling, addressing emotional, behavioural, and psychological concerns resulting from parental incarceration.
• Support parent-child reunification, ensuring the best interests of the child are prioritised.
• Organise and facilitate educational workshops and support groups to educate children and families, and provide a safe space for sharing experiences and insights.
• Offer crisis intervention and trauma-informed care to children and families facing acute challenges.
• Advocate for the rights and needs of children and families, coordinating with various organisations and systems to address systemic barriers.
• Provide supervision and guidance to junior social workers and caseworkers in case management and groupwork.

MKFC team

• Conduct initial and regular assessments on needs of children/youth
• Provide casework interventions and support for children, youth and their caregivers.
• Maintain updated case notes and social work reports.
• Schedule regular visits to clients & caregivers
• Plan, develop and deliver programmes based on the assessed needs of clients.
• Participate in individual and group supervisions
• Manage engagement and collaboration with other stakeholders, including volunteers and other agencies, to provide holistic support to clients
• Work in a team to support the smooth running of daily operations and maintenance of our MKFC centre
• Regular review of case management process and procedures to ensure these are in line with sector practices

Skills, Competencies and Qualification

• With working experience in children and youth work will be an added advantage.
• Confident in building relationships with children and youth.
• Able to collaborate with caregivers, volunteers, and other stakeholders such as school and community partners.
• Strong team player with good interpersonal and communication skills.
• Teachable heart and willing to embrace new learning opportunities.
• Must have the flexibility to work on Saturday mornings and weekday evenings (as needed).

• National Institute of Technical Education Certificate (NITEC) in Nursing or equivalent
• At least 4 years experience as an Enrolled Nurse for a Senior Enrolled Nurse
• Experience in ILTC setting is an advantage
• Valid Registered Nurse practicing certificate issued by Singapore Nursing Board
• Valid BCLS certification
• Comfortable with shift work (3-rotating shift) and open to deployment within 2 work sites under SingHealth Community Hospitals
• Uniform required

You will facilitate the continuity of care for patients throughout their hospitalisation and transition to home and community.

• Diploma or Degree in Nursing or its equivalent
• Advanced Diploma / Specialist Diploma / Graduate Diploma in Nursing (Gerontology / Orthopaedic / Neuroscience / Palliative Care / Community Nursing) is preferred for Senior Staff Nurse
• At least 3 years experience as Registered Nurse in acute hospital setting
• Experience in ILTC setting is an advantage
• Valid Registered Nurse practicing certificate issued by Singapore Nursing Board
• Valid BCLS certification
• Comfortable with shift work (3-rotating shift) and open to deployment within 2 work sites under SingHealth Community Hospitals
• Uniform required

As a valuable member of the SCH family, you are an integral part of the care team for rehab and sub-acute patients. You will assist the nursing staff in administrative duties, basic nursing care and infection control practices for an assigned group of patients. You will perform housekeeping duties, which includes ensuring a clean and tidy ward environment, as well as the maintenance of the ward inventory.

• GCE “N” Level
• A recognized Certificate in Health Care (Inpatient)
• Comfortable with shift work (3-rotating shift) and open to deployment within 2 work sites under SingHealth Community Hospitals
• Uniform required
• Passion to serve the community

The jobholder will assist the healthcare team in providing basic caregiving activities and engagement to patients to ensure comfort and patient safety under the supervision of trained nurses. He/she aims to establish positive rapport with patients to improve the patients’ hospital experiences and to deliver pyscho-social care for patients.

Job Requirements:

• Minimum GCE “N”/ ”O”/ ”A” levels with 4 years of general working experience
• Strong interpersonal and communication skills. Candidates with the ability to speak different dialects will be advantageous
• A good team player with the ability to work and collaborate well with colleagues across all levels
• Passion to care for people and strong ability to empathize with patients from all walks of life
• Able to perform 2 rotating shifts (AM and PM shifts) and open to deployment within 2 work sites under SingHealth Community Hospitals
