Healthcare Support Department supports frail/home-bound elderly in their healthcare needs so that they can remain to age well at the comfort of their homes and in the community. The department understands the challenges faced in taking care of elderly at home. They ensure there is continuity of care integrating healthcare and social support whenever necessary so that caregivers and their loved ones will be well-supported and enjoy good care at the same time.

Responsibilities:

• Nursing Assessment, developing individual careplan, monitoring and evaluation
• Assess, monitor, review care needs of client and caregiver
• Develop individualised care plan and identify types, duration and frequency of services
• Monitor client’s response to services and evaluate progress towards goals.
• Able to perform nursing procedures such as wound care, tube change, medication management and initiate medical care interventions where necessary
• Provide health education for the client and caregiver
• Provide caregiver’s training for the client’s caregiver
• Manage acute and chronic medical conditions in consultation with a doctor

Requirements:

• Minimum a Diploma in Nursing, at a local tertiary institution
• Minimum 1 year experience in nursing
• Registered with Singapore Nursing Board
• Possesses a valid certification in Basic Cardiac Life Support (BCLS)
• Good communication & interpersonal skills
• This position is on contract basis, with normal office working hours

Family Central, a service by Fei Yue Community Services, was officially appointed by the Council for Third Age (C3A) as programme organiser for the Intergenerational Learning Programme (ILP) with effect from June 2013.

The ILP aims to encourage intergenerational bonding by matching youths and seniors in a group learning environment. Through practical lessons such as Skype, Facebook, managing health and laughter yoga, seniors acquire new knowledge while youths share theirs and learn character building.

Family Central can provide a framework and methodology, capacity building through training, consultancy and sharing of best practices, and participating schools can count their ILPs as part of their service learning initiatives. These programmes will help forge intergenerational solidarity and promote social cohesion.

Responsibilities:

• Plan, develop and implement effective strategies to promote the Intergenerational Learning Programme (ILP) to seniors
• Recruit senior participants for the ILP workshops
• Process course registrations through the Training Management System (TMS)
• Manage administrative matters related to recruitment
• Assist Team Lead in preparing for the monthly report submissions
• Handle senior enquiries through phone calls, text messages and emails
• Travel down to the various schools within Singapore to oversee the ILP workshops
• Other duties as assigned by supervisors

Job Summary:

Your key responsibility will be to manage and contribute to a portfolio of projects comprising research studies, programme design, monitoring, and evaluation to improve programme efficacy and outcomes of beneficiaries.

Activities include project management, applying evidence in programme design, developing indicators for monitoring and evaluation, providing implementation support, conducting research studies and research synthesis. You will also contribute to strategic organisational development and capability building projects

Principal Responsibilities and Duties:

1) Support research studies and activities

• Conduct literature reviews
• Scope research studies and design surveys
• Prepare reports and present findings through reports, newsletters, presentation, etc.

2) Work with programme teams to design and develop evidence-informed programmes and interventions

• Project planning and management
• Building strong relationships with stakeholders
• Provide relevant evidence-based information
• Support in needs assessment/ situational analysis and synthesis
• Develop monitoring indicators and scales for programme monitoring and evaluation (both internal and external)
• Contribute to proposal writing
• Support teams in programme implementation to ensure monitoring process is in place

3) Work alongside teams for programme monitoring and evaluation (both internal and external)

• Co-ordinate and manage programme evaluation projects with external programme evaluators
• Support programme monitoring and evaluation of internal programme evaluations
• Prepare reports and present findings through reports, newsletters, presentation, etc.

4) Contribute to organisational capability building e.g. training workshops, etc.

• Support in preparing and conducting training on related impact and research topics

Essential Skills and Qualifications:

• A good degree in any discipline, preferably with prior experience conducting research programme design, monitoring and evaluation
• Excellent written and verbal communication skills
• Qualitative and quantitative research design, data collection and analysis skills
• Strong ability to interpret, analyse and synthesise data
• Strong project and stakeholder management skills
• Proficiency in using statistical software such as SPSS or Nvivo to conduct data analysis
