The Senior /Medical Social Worker Assistant supports and assists the Senior / Medical Social Worker (S/MSW) to look into the possible interventions (to receive social and financial assistance, renew subvention of healthcare subsidy and eligible funding schemes) to help reduce the burden of the clients and families from incurred social and healthcare costs.

Work Location: Pearl Hill Care Home

Responsibilities:

• Regular liaisons with clients and families to following up on pending assistance – Medifund, Household Means Testing (Subvention) and other funding schemes, etc.
• Be updated with available financial assistance available and provide advice to clients and their families on eligible resources to tap on
• Support and adhere to the care plans developed by S/MSW (Centre Manager) and other healthcare colleagues
• Monitor clients’ social wellness and participation rate in social activities during their stay
• Assist in follow-up and liaisons with community services in the discharge care plan of clients, if any.
• Financial counselling for all clients during assessment
• Closely monitor to ensure the meeting of KPI of 10 days for referral intake
• Provide administrative support by keeping records, filing and update progress reports, etc.

Requirements:

• Diploma in Social Work

General Duties

• Work in partnership with the SBP team.
• Work collaboratively with professionals from respective social work organizations, the MSF, educational institutions, hospitals and any other organizations involved in the care plan of every child.

Case Management

Resident Management

• Co-lead with Residential Care staff to ensure the resident’s safety, health & positive behaviour when they are in the Home and during outings & appointments.

1. Monitor the residents to ensure that they are present in SBP, house or the activity you are supervising (or their whereabouts are known to you when they are not at the Home) and double check, communicate and give permission for residents to leave areas or activities under your supervision when it is requested or required.
2. Ensure that they follow and adhere to the structure in SBP.
3. Monitor the residents to ensure that they are physically safe and do not partake in any risky behaviour or activities which may cause themselves or others physical injury – especially during playtime.
4. Monitor & report to the Management of SBP of any complaints from residents, family members, volunteers or visitors to the Home.
5. Monitor & report to the Management of SBP of any signs or suspicions of physical, sexual or emotional abuse or self-harm.
6. Monitor & manage the behaviour of the residents by encouraging & rewarding good behaviour and by preventing, discouraging or discipline misbehaviour in strict adherence to the rules & discipline procedures of the Home.
7. Supervise residents and teach rules and provide reminders about safe touch & unsafe touch; and check in with residents to flag any behavioural concerns and issues.

We are seeking a compassionate, resilient and competent Social Worker to join our team. The ideal candidate will be involved in casework management and working collaboratively with key stakeholders and colleagues to deliver trauma-informed practices to enhance the safety and well-being of our vulnerable women and children who have experienced family violence and trauma.

Roles and Responsibilities

• Possess at least a recognized Degree in Social Work
• Perform case management and counselling for our residents
• Able to plan and implement intervention programmes for our residents
• Conduct assessments for admission and discharge
• Prepare and adhere to the timely submission of required reports
• Possess good analytical, communication and interpersonal skills
• 4 days work-week, and include 1 weekend
• Willing to work in a residential setting
• An individual with at least 2 years of social work-related experience

• Evaluate and Assess specific care requirements of individual elderly and develop care plans tailored to their needs.
• Create and execute programs for elderly utilisiing data analysis to monitor the effectiveness and adjust according to the needs.
• Collaborate with various stakeholders to coordinate care, facilitate service referrals, activitly participate in case discussions to ensure comprehensive support.
• Implement and evaluate new activities, review and initiate area for enhancement.
• Mentor volunteers, manage budgets and resources, and collaborate with other organizations to deliver programs effectively.

Job Requirements :

• Diploma/Bachelor’s Degree in Social Work.
• Preferably relevant experience in Social Service sector, working with elderly clients or any related field.
• Proficient in Microsoft Office (MS Word, Excel and PowerPoint).
• Able to work independently and in a multidisciplinary team to meet deadlines.
• Good interpersonal and communication (oral and written skills).
• Possess some knowledge of programme design, implementation and evaluation.
• Willing to work Mon to Fri 9am - 6pm.

We are looking for a Medical Social Worker to join the team.

As a Medical Social Worker, you are responsible for providing psychosocial support for elders and caregivers through in-depth assessment, case management and multi-disciplinary approach to facilitate care and maximise elders’ dignity.

Responsibilities:

Care Coordination/Management

• Management of caseload requiring social work intervention within stipulated response time in assigned setting.

• Conduct BPSS assessment, case management and coordination of care for referred elders and caregivers.

• Collaborate with multi-disciplinary professionals to promote well-being of elders and caregivers through multidisciplinary meeting (MDM).

• Develop and coordinate discharge planning process for potential elders in transitioning from SLEC to other services if appropriate.

• Provide community case management services and make appropriate referrals to other agencies when required.

• Participate in case discussions.

• Carry out relevant tasks as assigned by clinical supervisor and/or Head of Department.

Programme

• Support and co-facilitate therapeutic groupwork/programs for elders and caregivers as appropriate to their needs.

• Facilitate ACP conversations when appropriate.

Admin

• Facilitate and prepare reports for financial assistance applications such as Medifund, Subsidy Deviation etc.

• Ensure proper documentation of case files, referrals and other related documents for case management and audit purposes.

• Contribute to half-yearly statistical report for funded program.

• Participate in quality improvement initiatives/projects.

• Sourcing community support networks and programmes for the benefits of our elders.

Interpersonal

• Ability to converse in local languages to be able to communicate with clients.

• Team player with good interpersonal, organizational, and communication skills.

• Good knowledge of Microsoft office.

• Able to work independently.

Job Requirements:

• Degree / Graduate Diploma in Social Work.

• Minimal 2 years of direct social work experience.

• Prior work with elders in a community or healthcare setting would be advantageous.
