Role Overview: As a Social Worker, you will be a vital part of our team, helping children in need. Your main jobs will include making personalized care plans, visiting families, and linking up with community help. Keeping detailed records and helping out with orphanage events and activities are other important aspects of this role. Responsibilities

Care Planning & Case Management

• Attend to pre-admission referrals and draw out Individualized Care Plans (ICPs and Needs Assessments for necessary intervention upon admission
• Schedule regular interactions i.e. visitation within families, siblings, or/and extended family members (or/and other caregivers)
• Make regular home visits with families or meetings with external/community resources i.e. schools, SSAs, the healthcare sector in gathering insights and recent developments/updates
• Work with families to equip them with the necessary skills and assistance to be able to manage and provide adequate care for their children (i.e. financial/legal assistance, job training/placement, parenting classes, substance abuse/mental health treatment etc.)
• Collaborate with various community partners in maintaining and reinstating familial ties for reintegration or/and preparation for self-efficacy in preparing for discharge
• Identify community-based support resources and ensure appropriate planning for independent living
• Assist with transitional placement of the children in securing other permanent/alternative care options
• Make referrals and introductions to other social service agencies for relevant services & programmes for both the children and families
• Offer information and support children and their families

Counselling

• Address the development, emotional and mental needs of the children in providing quality care for the children in residence
• Provide counselling to the children and initiate/coordinate support linkages for the children with internal/in-house programmes or/and external agencies including schools, mental health facilities, private practice etc
• Provide counselling to family members in fostering family bonds and restoring relationships

Administration

• Maintain accurate records and prepare social reports (pre-admission, case review/summary, care plans, discharge etc) as required
• Maintain computerized records of admission, discharge and case review of all children
• Ensure case file documents and recordings are kept up to date
• Prepare and submit monthly statistical reports to MSF and other relevant agencies
• Assist in the development of the Casework Management System

Other Duties

• To support MTFA / Darul Ihsan in all events/functions
• To perform any other duties assigned by the Home Manager or Executive Director

Candidate Requirements

• Degree in Social Work / Psychology / Counselling
• At least 2 years of casework and counselling experience
• Good care management and counselling skills
• Certified in doing mandatory counselling cases will be an advantage

It is a lean work team consisting of social work practitioners, who manage a diverse range of cases addressing issues including homelessness, financial hardship, employment difficulties, lack of access to education and basic needs, system barriers, caregiving stress, etc. The team also manage cases and referrals to and from other CWS centres, depending on the needs of the clients and relevant services catered to their needs. The team expects individuals to be self-motivated, have the flexibility and adaptability to operate in their work functions with minimal face-to-face supervision, and have the passion to serve clients in a timely and empathic manner.

This role is an opportunity for a Social Worker, who is passionate to contribute to empowering disadvantaged individuals and families in working towards improvement in their lives. In this role, you will be reporting to the Principal Social Worker and assigned supervisor, and work collaboratively with the social work team to work towards elevating professional standards and service delivery to better serve CWS’ clients. You will also be provided on-the-job training and regular clinical supervision to guide you in achieving key professional competencies.

You will:

- Work collaboratively with clients and community stakeholders, in a systemic approach.

- Use a trauma-informed care approach to engage clients/families professionally and therapeutically.

- Provide clients with relevant information and make referrals to external agencies.

- Manage cases that require one-off, short- to medium-term financial or social assistance.

- Conduct assessments for intake triage for CWS programs and shelter referrals.

- Conceptualize cases and co-construct intervention plans with clients, and recommend clients for the relevant assistance.

- Conduct face-to-face assessments, case sessions, make phone calls, home visits, and site visits as part of case management work.

- Respond to clients/Carry out interventions in a timely manner according to professional standards.

- Perform accurate documentation (i.e., case recordings, social reports) relating to case work and input in designated IT system in a timely manner.

- Advocate for clients in helping them overcome system barriers and ensure they have equal opportunity to access relevant social services with minimal prejudice.

Requirements:

- A *Degree/Graduate Diploma/Masters in Social Work.

- At least 2 years of relevant experience in a Family Service Centre, or any Social Service Agency.

- Good spoken and written English.

- Conversational in any second language/dialect.

- Willingness to comply with Social Work ethics.

- Willingness to work with vulnerable clients without prejudice, including but not limited to, elderly, homeless individuals, persons with mental illnesses and disabilities.

- At least moderate level of competency in operating IT systems (i.e., Microsoft Office, Outlook, Salesforce, Zoom). Minimally, you must be willing to learn and operate the mentioned IT systems independently.
