Job Summary: As the main contractor’s Safety Officer, you will report to the Project Manager and be responsible for maintaining a safe and compliant construction site. 1. Receive and assess referrals in conjunction with clinical team to determine suitability and ensure appropriate placement in Home/Centre. Make recommendations to ED or designate for approval of admission and funding.
2. Provide case management and psychosocial support services to residents, clients, and their families. Utilize appropriate frameworks for holistic assessment (e.g., BPSS model, Systems theory).
3. Identify and develop individualized care plans together with clinical care team to meet the psychosocial and emotional needs of residents and clients.
4. Address issues related to grief, adjustment, and coping with chronic illness or disability.
5. Implement and facilitate appropriate casework interventions, including ongoing monitoring and evaluation of outcomes.
6. Establish network with external partners and stakeholders for resourcing and ensure continuity of care of residents/clients across settings.

12. In compliance with government regulations for continued funding, conduct annual review of resident/client's socio-economic status and make appropriate recommendations for financial assistance (e.g., deviation, Medifund) if necessary.
13. Participate in multidisciplinary meetings and work closely with Case Manager (Clinical) in discharge planning reviews.
14. Attend case conferences, meetings, network with external agencies as required.
15. Be a resource person for specific projects as needed.
16. Undertake any other related duties considered necessary, as assigned by SWM/ASWM or equivalent, in support of the department or Home.

Qualifications
