The Community Resource, Engagement and Support Team (CREST) of REACH Community Services serves as a first mental health touchpoint in the community. We focus on raising public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals, provides emotional support to individuals and their caregivers and links individuals to relevant health and social care services when necessary. Actively, we conduct outreach events/activities to promote mental health awareness and identify and support those with mental health needs and their caregivers.

You are responsible for organizing and conducting outreach activities to increase mental health and dementia awareness, promote early identification, conduct assessment and provide casement management support for client care.

Key Responsibilities:

· Coordinate care and support to clients with or who are at risk of mental health issues and/or dementia with the appropriate resources.

· Assess and provide intervention to clients, including referring cases to other agencies as needed.

· Provide emotional support to clients and caregivers and maintain case documentation.

· Outreach and networking to the community and stakeholders to promote mental wellness and mental health awareness.

· Plan and conduct cognitive stimulation activities, mental health/wellness programs as well as dementia awareness.

· Organize caregiver group sessions and manage CSN-related administration and finances.

· Maintain accurate client records and program data in line with CREST guidelines as well as adherence to PDPA and security guidelines.

Requirements

· Possess at least a Degree in Social Work or Graduate Diploma in Social Work.

· Conversant in English and a 2nd language, ability to speak dialect is an added advantage.

· Good communication and interpersonal skills.

· A good team player, outgoing personality, dynamic and self-motivated with a strong passion for mental health needs.

RESPONSIBILITIES

About the Role

You will be part of our community outreach team, also known as Community Resource, Engagement & Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early recognition of at-risk individuals, providing emotional support to individuals and their caregivers and link individuals to relevant health and social care services where necessary.

• Provide case management services from assessment, case management, working with systems, case conferences, and closure and to maintain proper and up to date case documentation
• Plan, coordinate, execute and evaluate on-going programmes and services, including outreach, on-site/online public education
• Conduct analysis, develop and implement mental health programmes and services to meet the needs and trends of the community
• Attend to enquiries and provide information and referral services
• Engage key community stakeholders to coordinate and implement mental health programmes
• Coordinate with other professionals and mobilise community resources for service delivery
• Provide supervision to junior caseworkers. Volunteer management and providing supervision for volunteers and interns
• Upkeep programme data, records, and documentation. Prepare and submit periodic reports for key stakeholders
• Support other CCSS programmes as required. Opportunities to be involved in organizational level projects

Key responsibilities for Senior Social Worker:

• Lead conceptualizing, planning and implementation of CREST services in Toa Payoh (individual, group and community-based)
• Lead multi-disciplinary case conference as part of intervention process. To guide junior workers in facilitating case conferences of a similar nature
• Manage and supervise staff in provision of CREST services
• Conduct coaching and performance management review with staff
• Mentor, coach and lead staff in their practice and competency development
• Contribute to the Integrated Case Management Core Team in service planning and development
• Engage and collaborate with stakeholders to provide continuum of care, community mental health support and partnerships for social-health integration.

Bachelor/Post or Graduate Diploma in Social Work

Requirements:

• Passion for mental health and/or seniors work
• At least 5 years of relevant working experience with good knowledge in area of community mental health and have experience in conducting group and community based programmes; At least 3 years of supervisory experience to be considered for Senior Social Worker position
• Proficient in Microsoft Office
• High proficiency in written and spoken English with good facilitation and presentation skills; able to speak in mandarin and/or dialects would be essential for engagement with seniors
• Strong interpersonal and communication abilities with individuals and systems at multi-levels
• Good analytical and assessment skills
• Team player with an ability to function effectively in a fast-paced environment
• Strong organisation skills and structured.

Job Description:

• This role will require you to support GL, AP and AR activities, and to be part of the monthly closing process and other recurring accounting activities for the entity.

• Ensure timely, accurate month-end closing and financial reporting to Management.

• Responsible for preparation and closing of monthly and annual financial statements.

• Coordinate and liaise with internal parties to support intercompany activities.

• Coordinate and liaise with external parties like external auditors, tax agents, corporate secretarial agents, local tax and government authorities where required.

• Support and ensure compliance with all finance filing requirements, tax filing requirement (GST/WHT/CIT/…), audits, procedures, and forms both statutory and internal.

• Any other duties as assigned.

Requirements:

• Bachelor’s degree in one of: business, finance, accountancy or similar field. CA, ACCA, CPA or CMA Certification is preferred.

• 2 - 3 years of experience working in audit, accounting/finance-related or similar field.

• Advance understanding and knowledge of Accounting in Construction Industry

• Highly flexible, creative problem solver, with a strong ability to multi-task.

• High tolerance for ambiguity, changing work priorities and deadlines, and a willingness to take on responsibilities and to prioritize own work on multiple projects.

• Strong analytical and problem-solving skills, with the ability to analyse complex data, identify trends, and communicate insights effectively.
