Job Description & Requirements
Responsibilities
· To provide case management/ supportive counselling for seniors and their family caregivers. The social worker is to conduct psycho-social and financial assessments, including para-counselling. Makes appropriate recommendations and to carry out an effective clinical intervention for the clients.
· To plan, develop and implement programmes/services according to the needs of the seniors/community and family caregivers.
· To plan and facilitate in caregiver support group/ group work
· To conduct home and hospital visits
· To network and liaise with stakeholders, relevant agencies, and organisations regarding to the welfare of the seniors and their family caregivers
Requirements
· Must have a Degree or Post Graduate Diploma in Social Work or Masters in Social Work
· Minimum 3-year experience working in the social service sector, preferably in the senior care sector
· A team player and able to work independently
· Passionate about serving the social service sector to provide better services and care to the clients
· Possesses good attitude with good interpersonal and communication skills
· Bilingual and dialect speaking preferred to liaise with dialect speaking clients
· Proficient in Microsoft Office
· Open to Singaporeans only
