About the Role

You will be part of our community outreach team, also known as Community Resource, Engagement & Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early recognition of at-risk individuals, providing emotional support to individuals and their caregivers and link individuals to relevant health and social care services where necessary.

• Refer Authorised Occupiers to relevant external agencies and partners as necessary, including but not limited to the Ministry of Social and Family Development’s Social Service Office (SSO), Community Development Councils (CDC), Family Service Centres (FSC), Workforce Singapore (WSG), and / or community partners such as those in the SG Cares Community Network.
• Refer Authorised Occupiers to New Hope CS’s Employment Services team and Programmes in Kampung Siglap if relevant.
• The role of the Managing Agent does not include casework intervention but does include working closely with the assisting agencies / partners to help the Authorised Occupiers build stability and self-reliance.
• Work with other social service agencies and community partners.

2. Regular reports

• Submit regular reports to the management in accordance with HDB and organisation report.
• Submit reports to management and other government agencies as required.

3. Other organisational-related matters

• Participate in activities of New Hope CS (e.g. Fundraising, teambuilding,…etc)
• Support other services that New Hope CS provides e.g. Family Shelter, Individual Adults, HOPE Scheme, Children & Youth, Kampung Siglap Lifeskills Training and Retreat Centre…etc
• Perform any other related duties as assigned.

5. Candidate Requirements

Technical Competencies:

• Degree in Social Work from an accredited institution.
• Accredited as Registered Social Worker with SASW.
• At least 2 years in direct social work is preferred.

Core Competencies:

• Strong interpersonal and communication skills.
• Understands the principles of engaging and working with vulnerable families and individuals.
• Case management and casework knowledge and skills.
• Risk assessment and risk management.
• Good report writing skills.
• Willing to work with the homeless and destitute.

Job Description

• Render professional psycho-emotional, social and spiritual care and support to patients and their significant others and, in doing so, help them lead more meaningful and satisfying lives in the context of the situation where the death of a family member is imminent.
• Play an active role in the planning and execution of the activities of the Hospice that promotes the objectives and the mission of the Hospice.
• Assist in the development of policies and protocol related to the delivery of clinical services and recommend organizational and system changes in order to position the hospice as a leader in hospice palliative care.
• Engage, train and supervise volunteers in specific areas of patient care together with the Volunteer Programme Manager.
• Organise and conduct workshops for community partners.
• Promote the practice and use of palliative care by undertaking educational, research and outreach activities as assigned from time to time.
• Be prepared for job rotation around the different programmes or services, e.g. Inpatient, Home Care and Programme Dignity to address different patients’ needs and optimise manpower resources.
• Participate in the organisation of memorial service, support groups, outings and any other ad-hoc projects assigned by management.
• Any other duties as assigned by the supervisor and management from time to time.

Requirements

• Degree in Social Work / Counselling or relevant formal qualifications from a recognised university.
• Preferably 1-2 years of relevant experience in a healthcare environment.
• Good Interpersonal and communication skills.
• Energetic, committed and enjoy working as a team.
