As a Social Worker within the agency, you may be deployed to work with various client groups, including families, elderly, children and youth.

While this is a full-time position, if you’re unable to commit to full time employment but can dedicate 16 to 20 hours per week, we’d be happy to have you join our team as a locum.

Deployment Opportunities

As a Social Worker within the agency, you may be deployed to work with various client groups. We have opportunities in various services which include Individuals & Families, Children & Youth, Mental Health and Seniors.

Roles & Responsibilities:

The roles & responsibilities of the social worker would include but are not limited to the following:

• Conduct triage and intake services for members of the community.
• Provide casework and counselling interventions for individuals and families.
• Plan and conduct group work to support the needs of beneficiaries.
• Engage in community work to build an ecosystem of assets and resources.
• Engagement and advocate with systems and stakeholders in the community.
• Strategize and coordinate interventions to support beneficiaries.
• Assist with company projects or report submissions.
• Carry out any other duties as assigned by HOD/supervisor.

Candidate Profile:

• Bachelor / Postgraduate Diploma / Masters in Social Work.
• Experience in one or more of the following areas:

1. Casework
2. Counselling
3. Risk and crisis management
4. Group work facilitation

• No prior work experience required for new entrants to social work.
• Strong analytical skills with the capability to identify and solve complex problems in a timely, efficient manner.
• Exceptional interpersonal skills that enable seamless collaboration with diverse groups.
• Excellent communication skills with the ability to listen actively, articulate ideas concisely and collaborate effectively with team members.
• Passionate about uplifting vulnerable individuals and building thriving communities.

Job Summary

The Social Worker provides bio-psych-social assessment and intervention in order to create a sustainable health and social support network for the elderly and their families.

Key Responsibilities:

• Conduct pre-admission assessment of referrals for admission to programme
• Provide case management support through assessment, case planning and interventions.
• Work within a multidisciplinary framework to develop individualised client’s care plan with different team members
• Implement components of care plan relating to support for the caregiver and family members
• Liaise and network with relevant social and community service agencies to establish or enhance the clients’ social support system, enabling them to remain living within the community Key

Requirements:

• Degree / Graduate Diploma in Social Work with minimum 5 years of working experience, preferably in eldercare or related fields
• Accreditation with the Social Work Accreditation Board
• Experience with care integration and case management in an allied health setting will be advantageous
• Competent in administration and organisation of case notes and recordings
• Able to work independently and as part of a multidisciplinary team
• Penchant for working with the elderly and conducting home visits
• Good communication and writing skills
• IT proficient
