• Salary: Up to $3,000 for candidates with relevant diplomas and internships; up to $3,300 for those with prior experience in Social Work Associate roles.

Key Responsibilities:

• Support Clinical Team: Help clients apply for financial schemes, update renewal applications, and assist in administrative processes.
• Onboarding Process: Manage onboarding of new clients, including scheduling and assisting with subsidies and rehabilitation inquiries.
• Basic Case Management: Help assess client and caregiver needs, with guidance from case managers or social workers.
• Referral Assistance: Assist clients and caregivers in accessing appropriate community services and support.
• Collaboration: Work with multidisciplinary teams and assist with joint home visits and case discussions.

Qualifications:

• Minimum Requirements: Diploma in healthcare management, counseling, psychology, or related fields; internship experience can be considered.
• Experience: Previous experience as a Social Worker Associate or in a similar role can warrant a higher salary offer.
