As part of the national HealthierSG initiative to transform the way healthcare is delivered, NUHS Regional Health System Office (RHSO) is setting up a Care and Call Centre (CareHub) to support the western population through phone calls, telemedicine and digital communication platforms to become healthier and take charge of own health.

You will be responsible to helm CareHub in line with CareHub’s service standards and key performance indicators (KPIs) and deliver a positive customer experience for residents and community partners who reach in to CareHub for support to age well in the community. As a Care Associate, you will be part of an integrated care team that delivers a person-centered care model for our seniors, including persons with dementia. You will need to:

• Perform activities for daily living (ADLs), including assistance with personal hygiene, mobility, toileting, diaper changing, and feeding
• Monitor seniors' medical and health conditions
• Facilitate small group activities and exercises catered to the seniors' interests
• Engage seniors, including those with dementia, in meaningful and stimulating activities that promote mental and emotional well-being.
• Perform general housekeeping to maintain the cleanliness and hygiene of the centres
• Escort and assist seniors during transportation to and from the care centre ensuring their safety and comfort throughout the journey

Required Qualifications & Skills:

• GCE N' or O' Level
• PSLE education certification would require an additional International English Language Testing System (IELTS) certification
• Certificate/ training in healthcare or equivalent

You should:
