A. Programme Development and Implementation
i. Provide triage, care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.
ii. Followed up with assessment and review information and referral service and monitoring of clients with mental health condition
iii. Plan and conduct talk and outreach to the community and stakeholders to promote mental wellness and mental health awareness, including community mapping, deck of presentation
iv. Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
v. Plan and organize structured preventive activities and/or programmes for the CREST clients and community, those at risk of mental health conditions and clients.
B. Administration and Staff Development
i. Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance with the CREST programme requirement.
ii. Keep and maintain client and programme data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
iii. Review and analyse collated data and monitor Participate in Support in the development of CREST service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up).
iv. Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.
v. Participate in internal meetings and all SAGE events
vi. Attend required training from AIC for CREST Programme
C. PROFESSIONAL / CLINICAL
i. Ensure face-to-face counselling, home visits and other intervention such as telephone counselling, centre-based counselling following the guideline of counselling SOP
ii. Ensure client are satisfied with service and able to meet all the goals of counselling most of the times
iii. Ensure planning and execution of clinical-based and/or psycho-educational group work for clients and their family members.
iv. Ensure all case recordings are updated and all case statistics are accurately captured in a timely manner.
v. Ensure linking to relevant community based/clinical assistance to clients or their family members in need of help.
vi. Ensure coming to regular internal or external clinical supervision prepared and be able to reflect on practice
D. Other Duties
i. Assists in Public Education, outreach and other Centre activities of SAGECC
ii. Any other relevant/appropriate duties as assigned by Centre Manager or Executive Director.
