Job Description

As a Community Care Associate, you are a vital member of a dedicated care team, ensuring that our seniors leave our centres happy and eager to return. Through our person-centered care model, you will promote seniors' independence and stimulate their cognitive and physical functions while attending to their care needs. With support and training from our team of professionals, you will continue to grow in your role. We are here to help you grow in your career and achieve your full potential in a nurturing and collaborative environment.
You will need to:
- Assist in serving pre-packed medication
- Assist in client health monitoring and assessment
- Aid in meal preparation and serving
- Assist in the implementation and monitoring of the Initiate Care Plan (ICP)
- Provide dysphagia care and EatSafe protocol
- Support clients in distress including persons living with dementia
- Assist in the Modified Barthel Index (MBI) assessment
- Plan and conduct structured and interactive activities
- Perform basic Activities of Daily Living (ADL)
- Assist with Passive Range of Motion exercises (PROM)
- Implement and support individual therapy
- Perform escort duties and build rapport with clients and their family members
- Assist in cleaning and disinfecting tasks
- Participate in and support centre events
Qualifications
- C6 pass at GCE N' Level in any 1 subject
- WPLN 4 (Reading, Listening, Speaking) and WPLN 3 (Writing)
- Some experience in social service or the healthcare industry would be an advantage
- Facilitation skills and healthcare knowledge would be an advantage

Working hours:
- Five-day work week (centre operating hours are from 7am to 7pm)
- The expected shift hours are from 7am to 4.30pm, 7.30am to 5pm, 8am to 5.30pm, 8.30am to 6pm, 9am to 6.30pm and 9.30am to 7pm (42.5 working hours per week)
