Scope and Responsibility :

Radiographer is responsible for the providing prompt diagnostic imaging services to the patient, ensuring patient safety and proper care of equipment. Assist Centre In-charge/ Senior or Principal Radiographer to guide and supervise junior and student radiographers when required.

Job Responsibilities:

• Perform general radiographic examinations and/or at least one of the area of specialty including mammography and/or ultrasound and/or Bone Mineral Densitometry.

Overview

As Local Coordinator and as part of the Blended Care Team, you will assist the care management team in developing social activities and interest groups for residents at a physical centre, under the Health District @ Queenstown. This effort is in part to support social-health integration and enhance care delivery in the community.

These activities include working with residents and community partners to co-create, plan and conduct ground-up activities (e.g. exercise groups, hawker centre tours, etc.), encourage residents to participate in suitable programmes, within and beyond the physical space and oversee the general operations at the centre.

You will be involved in facilitating the development and implementation of Health District Queenstown Lighthouse Projects involving National University Health System (NUHS) and various community partners in Queenstown to support social-health integration and enhance care delivery in the community.

Job Responsibilities

You will be responsible for the following:

1. Co-create and organise activities and interest groups at the physical space

· Engage residents and volunteers to co-create, plan, and conduct activities and interest groups e.g. hawker centre tours to share on healthier eating habits.

SAMH SAY-IT! (Sports, Arts, Youth Integrated Team) provides assessment and intervention for youths, and engages youths for ongoing monitoring and support. YIT also works actively with youth agencies, schools and community partners to provide holistic case management for youths and their families.

Responsibilities:

Basic Emotional Support

· Manage client through client engagement, information gathering,case assessment (BPSS, risk & needs assessment and other relevant assessment tools), case conceptualization, case planning and intervention plans for best clients’ outcomes

· Provide supportive counselling to clients through face-to-face and/or

online means

· Conduct visits to school/centre, home or hospital

· Advocate for the protection of client's dignity and rights

Screening

· Screen referred young persons for mental health distress

· Recognise different levels of distress and provide relevant resources and basic supportive counselling via phone and face-to-face

· Identify presenting/underlying issues so as to design a care plan to enhance/maintain and promote psychosocial functioning of clients

· Link youths or caregivers with appropriate services for continuity of care

Programming

· Conduct risk assessments and ensure safety plans

· Monitoring youths at risk of mental health issues

· Source for and liaise directly with persons-in-charge of various agencies or groups who provide youth volunteers

Administration and Evaluation

· Administer K10 evaluation to youths and caregivers, and ensure proper documentation

· Assist Head and Admin Officer in Annual Report or statistics collation when necessary

Operations

· Coordinate and design monthly programme calendar

· Manage day-to-day operations of centre (e.g. opening and closing of centre, stocktaking, maintaining walk-in area and displays,overall crowd control)

· Coordinate and attend to all visits to the centre

· Report and follow up on any faulty equipments or furniture promptly to ensure smooth operation of the centre

· Ensure maintenance of premises and property

Other Duties

· To work on ad-hoc SAMH projects, community outreach programmes and events, ie World Mental Health Day, YOUth Alive!,etc.
