MAJOR DUTIES AND RESPONSIBILITIES

A. SPECIFIC (80%)

• Be certified and trained in skills as an ACP Facilitator and ACP Trainer.
• Facilitate ACP discussions with patients/individuals, families and NHS.
• Document these discussions and the wishes of the patient/individual in the appropriate forms (these may be hard copies or electronic forms).
• Update and liaise with primary care physicians on patient’s advance care plan.
• Coordinate between patients/individuals, relatives, and healthcare team for ACP facilitation and to follow-up with the necessary appointments for them.
• Mentor, train and guide junior ACP Facilitators in ACP case management, documentation and communication skills.
• Develop and improve ACP training curriculum and programmes e.g. Advocacy, General ACP and Preferred Plan of Care (PPC) Facilitation courses.
• Be involved in grief bereavement work and provide psychosocial support to patients and where appropriate, follow up with referrals to spiritual and/or psychiatric support.
• Develop and be involved in the spiritual care work and support for ACP patients and their family members.

B. GENERAL (20%)

• Support or conduct ACP awareness talks, road shows and training.
• Work in collaboration with key stakeholders in the evaluation and quality improvement processes for the ACP program.
• Participate in conducting audits, research and surveys related to ACP. Where necessary, staff will be nominated as co-investigators of research projects and are expected to be involved in data collection, abstract and paper writing etc.

EDUCATION, TRAINING AND EXPERIENCE

• A Degree with 5 to 6 years of working experience
• Strong communication skills, motivated, resourceful and a good team player
• Experience in ACP facilitations for 2 to 3 years in any setting

B. PROFESSIONAL LICENCE

• Nursing or allied health (social work etc.) background preferred.

ABNORMAL PHYSICAL WORKING CONDITION

• Staff may be required to travel to patients’ homes, other step-down care units or other locations as needed to conduct ACP discussions.
• Staff may be exposed to patients and their family members of various psychological and emotional states and who possess varying personality traits.

Associate Medical Laboratory Technologists (AMLT) are responsible for the daily operations of the laboratory which include phlebotomy, electrocardiogram, spirometry and clinical laboratory work.

Responsibilities

• Perform daily operations of the laboratory
• Perform all procedures in accordance with laboratory standard operating procedures (SOPs)
• Perform phlebotomy, Electrocardiogram (ECG) and spirometry in an accurate, efficient and professional way.
• Perform Point of Care Testing (e.g. Prothrombin Time, glucose) and clinical laboratory tests such as urinalysis (e.g. Urine FEME, dipstick, pregnancy), haematology (e.g. full blood count, peripheral blood film, erythrocyte sedimentation rate, prothrombin time), biochemistry (e.g. HbA1c, neonate bilirubin), microscopy examination (e.g. fungal smear, high vaginal swab) independently.
• Ensure specimen identification, processing and storage of test specimens are in consistent with good laboratory practices
• Perform and verify quality control for all analysers prior to patients sampling
• Use of Laboratory Information System (LIS) to perform functions such as result entry, result validation and tracking for outstanding tests
• Ensure all test results provided for clinicians are accurate and reliable
• Ensure analysers/equipment are functioning properly and are well maintained.
• Notify medical technologist/lab in charge when instrument malfunction and make arrangements with BME or instrument vendor to service the instrument
• Notify inventory person in charge when reagents and stocks supplies run low.
• Perform and record internal and external quality controls and calibrations. Troubleshoot outliers in accordance to work instructions.
• Ensure ‘critical value’ are reported to the relevant personnel within 30 minutes
• Ensure daily maintenance and QC records are up to date.
• Ensure compliance to hand hygiene, workplace safety, International Patient Safety Goals (IPSG) and International Standardization of Organizations (ISO) and other applicable regulatory and accreditation requirements of the laboratory.
• Provide prompt and courteous service in person or over telecommunication to all external and internal customers (e.g patients, doctors, service providers, colleagues).
• Report adverse incidents and all workflow anomaly to supervisor
• Observe strict NUHS personal data protection act and observe internet/intranet cybersecurity guidelines.

Requirements

• Recognized Diploma in Biomedical Science, Clinical Laboratory Science or other relevant fields
• At least 1-3 years of experience in laboratory environment and/or relevant experience in phlebotomy preferred
• Good verbal and written communication skills
• Ability to communicate in vernacular languages are an added advantage
• Ability to multi-task and work in a fast-paced environment
• Team player with good interpersonal skills

Radiographer is responsible for the providing prompt diagnostic imaging services to the patient, ensuring patient safety and proper care of equipment. Assist Centre In-charge/ Senior or Principal Radiographer to guide and supervise junior and student radiographers when required.

Job Responsibilities:

• Perform general radiographic examinations and/or at least one of the area of specialty including mammography and/or ultrasound and/or Bone Mineral Densitometry.
• Responsible for the safety and welfare of the patient.
• Maintain a clean and safe environment; adhere to safe work practices especially in radiation safety and infection control.
• Ensure proper usage and care for all equipment at the specific diagnostic centre.
• Perform other administrative duties or ad-hoc quality departmental projects/activities as assigned by the reporting officer/ Lead Radiographer
• Apply the ALARA principle for all procedures.
• Responsible for general housekeeping of the allocated diagnostic centre.
• Render prompt and courteous service delivery to external and internal customers.
• Ensure efficient running and operations of their area of responsibility.
• Assist the Radiographer-In-Charge / Senior or Principal Radiographer to ensure quality control procedures are performed diligently.
• Ensure AHPC certification is achieved after one / two years depending on qualification(s).

• Professional Licence: R1 license with Centre for Radiation Protection and Nuclear Sciences, National Environment Agency.
• Timely update and renewal of RI license.

Overview

As Local Coordinator and as part of the Blended Care Team, you will assist the care management team in developing social activities and interest groups for residents at a physical centre, under the Health District @ Queenstown. This effort is in part to support social-health integration and enhance care delivery in the community.

These activities include working with residents and community partners to co-create, plan and conduct ground-up activities (e.g. exercise groups, hawker centre tours, etc.), encourage residents to participate in suitable programmes, within and beyond the physical space and oversee the general operations at the centre.

You will be involved in facilitating the development and implementation of Health District Queenstown Lighthouse Projects involving National University Health System (NUHS) and various community partners in Queenstown to support social-health integration and enhance care delivery in the community.

Job Responsibilities

You will be responsible for the following:

1. Co-create and organise activities and interest groups at the physical space

· Engage residents and volunteers to co-create, plan, and conduct activities and interest groups e.g. hawker centre tours to share on healthier eating habits.

· Encourage and support residents to self-run activities and interest groups

· Partner with local community leader(s) and stakeholders to organise programmes and services

· Organise regular resident engagement sessions to gather feedback on activities and services provided by the Blended Care Team

· Develop and implement strategies to raise awareness of the space amongst Queenstown residents

2. Encourage residents to participate in suitable activities and programmes

· Encourage and direct residents to participate in suitable health and social activities (e.g. exercise, volunteerism, interest groups) at the space and in the community

· Coordinate with the Blended Care Team to ensure residents’ health and social care needs are met

3. Facilitate development of the physical space and oversee general operations

· Facilitate the conceptualisation and development of the physical centre

· Oversee general operations of space, and support in logistics and budgeting matters

· Record attendance and other data for evaluation

Requirements

· Bachelor Degree in the field of Social Sciences (e.g. social work, counselling, psychology), with prior experience in related areas such as social care services, aged care services and/or community engagement.

· At least 2 – 3 years of working experience.

· Ability to work independently, adaptable, analytical and a good team player.

· Strong communication and stakeholder management skills, and able to engage with partners and older adults in the community.

· Good organisational, time management, administrative and interpersonal skills.

· Good written and verbal communication skills in multiple languages and dialects is preferred.

Overview

As a member of Precinct Planning and Development Department under Regional Health Systems Office (RHSO), you will be involved in development of healthy precincts in the Western Region of Singapore in collaboration with key internal and external stakeholders and partners.

Overview and purpose of the position

The incumbent be responsible to operationalise engagement and operational strategies in support of MOH Healthier SG /Age WELL SG and play a role in the crisis response team.

• To facilitate and create a smooth and seamless process to enrol eligible residents on Healthier SG

• To render assistance in residents in resetting of Singpass

As a Senior Executive/Assistant Manager, you are responsible to support the day-to-day operations of the Care and Call Centre (CareHub) to meet CareHub’s service standards, key performance indicators (KPIs) and deliver a positive customer experience for residents and community partners who reach in to CareHub for support to age well in the community.

KKH is an 830-bed tertiary-level care hospital that provides obstetric, gynaecological, paediatric and other services. It is part of the SingHealth group of public hospitals. As an academic medical centre, KKH is a teaching hospital for Duke-NUS Graduate Medical School, Yong Loo Lin School of Medicine and Lee Kong Chian School of Medicine.
