This role is based in the polyclinic and you will be part of the multi-disciplinary team involved in the care of patients with chronic conditions, specifically diabetes. You will play a critical role in patients’ care journey and this includes engaging patients in the self-management of their chronic conditions and being their supporter/motivator. You will also be expected to take on some administrative functions to support the implementation team.

Your main duties will include the following:

• Collaborate and cooperate with physicians, nurses and other health care providers in the delivery of patient care and services.
• Preparation of medical reports/letters to be mailed to patients.
• Collection of patient data and maintenance of patient records/databases.
• Perform consent-taking, and administration of surveys/questionnaires.
• Perform accurate documentation in the delivery of patient care.
• Track, recall and reschedule patients who have defaulted their appointments
• Make appropriate referrals to care managers and community partners for interventions according to patients’ needs.
• Engage/communicate with patients on community programmes.

Key Responsibilities:

Data and Information Management

· Update clientele data and programme information

Outreach Support
