Scope of work:
Incumbent is responsible for providing assessment and design for Physiotherapy/Occupational therapy (PT/OT) programmes in collaboration with Nursing Manager.
Therapy services:
· Assess client and design PT/OT programmes.
· Conducting activities and exercises with Residents.
· Take note of residents’ progress and report progress to therapist / Nursing Manager written reports.
· Design and supervise the conduct rehabilitative work-orientated programmes based on assessment.
· Ensure that all approved programmes designed are carried out as per plan and endorse the assessment forms prepared to be submitted to Head of Home/Deputy Head of Home on monthly basis.
· Assist Home to conduct group activities for selected residents.
· Ensure safety and security of resident duringsessions.
· Prepare regular reports to update Head of Home/Deputy, Nursing Manager and Social Worker on functional status of residents.
Facilities upkeep:
· Maintain a clean and proper storage of all equipment and materials.
· Assist therapist monitor /maintain order of equipment /materials during sessions.
· Ensure safe usage of the equipment by residents and restrict use of damaged equipment.
· Highlight any equipment / property defects to Maintenance Officer.
· Provide feedback on facilities in the Home that need to be altered for use by residents or that affect safety of the residents.
Care team member:
· Provide inputs to preparation of Individualised Care plan of residents and monitor their progress along the milestones established on monthly basis
· Prepare feedback, wherever necessary, to external agencies /therapists on individual resident functional progress in the Home.
To undertake such other duties as assigned from time to time by the Head of Home/Deputy
Qualification and Experience:
Relevant certification or qualification in therapy services, therapy support, community care and services or any related fields. Registration with the Allied Health Practitioner Certificate, MOH is compulsory.
At least 1-2 years of experience in similar role. Incumbent must be confident of transfer techniques, such as lift and transfer resident from bed to wheelchair, wheelchair to chair.
Skills & Knowledge Requirements:
· Good verbal and written communication skills, including ability to effectively communicate with elderly residents, staff and external parties.
· Good computer proficiency (MS Office – Word, Excel).
· It is desirable for the incumbent to have the ability to work under pressure, while maintaining a positive and courteous attitude and providing exemplary resident care.
· Ability to work independently as well as part of the care team looking into the well-being of the resident.
· Good work ethics, be organised and punctual with strict adherence to Home policies.
· Able to communicate with residents in English, Malay, Hokkien, Tamil, Cantonese and Mandarin
Must have prior experience working in a similar capacity in Singapore.
