MAJOR DUTIES AND RESPONSIBILITIES

A. SPECIFIC (80%)

• Be certified and trained in skills as an ACP Facilitator and ACP Trainer.
• Facilitate ACP discussions with patients/individuals, families and NHS.
• Document these discussions and the wishes of the patient/individual in the appropriate forms (these may be hard copies or electronic forms).
• Update and liaise with primary care physicians on patient’s advance care plan.
• Coordinate between patients/individuals, relatives, and healthcare team for ACP facilitation and to follow-up with the necessary appointments for them.
• Mentor, train and guide junior ACP Facilitators in ACP case management, documentation and communication skills.
• Develop and improve ACP training curriculum and programmes e.g. Advocacy, General ACP and Preferred Plan of Care (PPC) Facilitation courses.
• Be involved in grief bereavement work and provide psychosocial support to patients and where appropriate, follow up with referrals to spiritual and/or psychiatric support.
• Develop and be involved in the spiritual care work and support for ACP patients and their family members.

B. GENERAL (20%)

• Support or conduct ACP awareness talks, road shows and training.
• Work in collaboration with key stakeholders in the evaluation and quality improvement processes for the ACP program.
• Participate in conducting audits, research and surveys related to ACP. Where necessary, staff will be nominated as co-investigators of research projects and are expected to be involved in data collection, abstract and paper writing etc.

ABNORMAL PHYSICAL WORKING CONDITION

• Staff may be required to travel to patients’ homes, other step-down care units or other locations as needed to conduct ACP discussions.

As a Senior Executive/Assistant Manager, you are responsible to support the day-to-day operations of the Care and Call Centre (CareHub) to meet CareHub’s service standards, key performance indicators (KPIs) and deliver a positive customer experience for residents and community partners who reach in to CareHub for support to age well in the community.

You will work closely with stakeholders and the CareHub team including Call Centre Officers (CCOs) who are helming CareHub, to draft, develop and implement requirements (including IT systems) and collaborative workflows and SOPs to bring plans into reality dovetailed to implementation timeline, organisation’s strategies, priorities and national’s directives. You will ensure that CCOs are properly trained and supported by a relevant knowledge management system to support handling of enquiries in the most professional and timely manner.

Associate Medical Laboratory Technologists (AMLT) are responsible for the daily operations of the laboratory which include phlebotomy, electrocardiogram, spirometry and clinical laboratory work.

Responsibilities

• Perform daily operations of the laboratory
• Perform all procedures in accordance with laboratory standard operating procedures (SOPs)
• Perform phlebotomy, Electrocardiogram (ECG) and spirometry in an accurate, efficient and professional way.

Overview

As Local Coordinator and as part of the Blended Care Team, you will assist the care management team in developing social activities and interest groups for residents at a physical centre, under the Health District @ Queenstown. This effort is in part to support social-health integration and enhance care delivery in the community.

These activities include working with residents and community partners to co-create, plan and conduct ground-up activities (e.g. exercise groups, hawker centre tours, etc.), encourage residents to participate in suitable programmes, within and beyond the physical space and oversee the general operations at the centre.

Job Responsibilities

You will be responsible for the following:

1. Co-create and organise activities and interest groups at the physical space

· Engage residents and volunteers to co-create, plan, and conduct activities and interest groups e.g. hawker centre tours to share on healthier eating habits.

· Encourage and support residents to self-run activities and interest groups

· Partner with local community leader(s) and stakeholders to organise programmes and services

· Organise regular resident engagement sessions to gather feedback on activities and services provided by the Blended Care Team

Requirements

· Bachelor Degree in the field of Social Sciences (e.g. social work, counselling, psychology), with prior experience in related areas such as social care services, aged care services and/or community engagement.

· At least 2 – 3 years of working experience.

· Ability to work independently, adaptable, analytical and a good team player.

· Strong communication and stakeholder management skills, and able to engage with partners and older adults in the community.

· Good organisational, time management, administrative and interpersonal skills.

Job Description

KKH is an 830-bed tertiary-level care hospital that provides obstetric, gynaecological, paediatric and other services. It is part of the SingHealth group of public hospitals. As an academic medical centre, KKH is a teaching hospital for Duke-NUS Graduate Medical School, Yong Loo Lin School of Medicine and Lee Kong Chian School of Medicine.

The position is based at the Department of Pathology and Laboratory Medicine (DPLM), the team is looking for pathologists with subspecialty interests in gynaecological pathology, breast pathology, cytology or any combination of the aforementioned areas. These are permanent positions affording the successful candidates the opportunity to work in a dynamic, friendly and supportive department.

The gynaecological, breast pathology and cytology services have six pathologists responsible for providing a comprehensive clinical service comprising gynaecological and breast surgical pathology, frozen section cover, cytology and multidisciplinary meetings. The role of the successful candidate can be primarily clinical with a component of education or research responsibilities in accordance to the candidate’s preferences and aptitude.
