Job Summary:
The Senior Care Coordinator will lead a dedicated team in providing comprehensive care coordination and case management services to patients across inpatient, outpatient, and community care settings. This role involves planning and implementing policies and procedures that support caregivers, enabling them to better assist patients.
Responsibilities and Duties:
Specific Responsibilities:
- Conduct home visits to deliver psychoeducation, support, and care coordination services to clients and caregivers.
- Assess patients' medical, psychological, social, environmental, and vocational needs.
- Collaborate effectively with a multidisciplinary team, including allied health professionals, doctors, and nurses.
- Provide psychological expertise during multidisciplinary meetings.
- Facilitate training sessions for team members and caregivers as needed.
- Execute projects and responsibilities assigned by the supervisor or Head of Department.
General Responsibilities:
- Develop and implement care management tools to enhance patient coordination.
- Identify areas for improvement within the department and hospital, recommending actionable changes.
- Assist in preparing reports and maintaining necessary statistics.
Job Requirement:
- Registered Nurse with valid certification from the Singapore Nursing Board.
- Possess a Registered Mental Nurse (RMN) or Advanced Diploma in Mental Health, or relevant qualifications in social work, counseling, or psychology.
- Minimum of 3 to 5 years of experience in psychiatric settings (e.g., Community Psychiatric Clinic, Institute of Mental Health) preferred.
- Strong passion for community work and supporting individuals with mental health disorders.
- Excellent organizational, interpersonal, communication, and writing skills.
- Proficient in computer literacy, including MS Office and other relevant software.
- Familiarity with the healthcare environment and mental health services.
- Ability to multi-task, work independently, and collaborate effectively within a team.
