RESPONSIBILITIES
About the Role
You will be part of our community outreach team, also known as Community Resource, Engagement & Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early recognition of at-risk individuals, providing emotional support to individuals and their caregivers and link individuals to relevant health and social care services where necessary.
- Lead conceptualizing, planning and implementation of CREST services in Toa Payoh (individual, group and community-based)
- Lead multi-disciplinary case conference as part of intervention process. To guide junior workers in facilitating case conferences of a similar nature
- Manage and supervise staff in provision of CREST services
- Conduct coaching and performance management review with staff
- Mentor, coach and lead staff in their practice and competency development
- Contribute to the Integrated Case Management Core Team in service planning and development
- Engage and collaborate with stakeholders to provide continuum of care, community mental health support and partnerships for social-health integration.
QUALIFICATIONS
Bachelor or Post Graduate Diploma in Social Work
Requirements:
- Passion for mental health and/or seniors work
- At least 5 years of relevant working experience with good knowledge in area of community mental health and have experience in conducting group and community based programmes
- At least 3 years of supervisory experience
- Proficient in Microsoft Office
- High proficiency in written and spoken English with good facilitation and presentation skills
- Able to speak in Mandarin and/or dialects would be essential for engagement with seniors
- Strong interpersonal and communication abilities with individuals and systems at multi-levels
- Good analytical and assessment skills
- Team player with an ability to function effectively in a fast-paced environment
- Strong organisation skills and structured.
