Responsibilities and Competencies:
• Conduct admissions assessments and means testing for new applicants, and provide orientation for newly admitted residents.
• Coordinate programs and activities that contribute to the residents' overall well-being, including medical appointments, medication administration and individualized care plans.
• Collaborate with community partners to effectively address the diverse needs of seniors.
• Liaise with next of kin (NOKs), authorities, medical institutions, etc.
Job Requirements and Qualifications:
• Diploma or Degree or equivalent professional qualifications.
• Preference will be given to candidates with a background in Nursing, Social Work, Allied Health, or related fields.
• Minimum of 3 years of experience in social services, community, eldercare, or hospital settings. Interns are welcomed to apply too
• Strong communication and interpersonal skills.
• Demonstrates genuine compassion and commitment to caring for seniors.
• Proficient in multitasking and time management, skilled at prioritizing tasks.
• Team player with the ability to collaborate with internal and external partners.
• Good computer skills and proficiency in MS Office applications.
• Able to work on weekends and Public Holidays, if needed.
