Job Summary:

The incumbent will carry out basic case management work and apply intervention strategies appropriately, under the guidance of the supervisor, to improve the well-being of clients so that they may age in place in the community. He or she will also be involved in the centre’s community outreach and public education initiatives as well as support the centre’s volunteer management initiatives.

Duties and Responsibilities:

1. Provides a person-centered case management services to vulnerable elderly living in the community which includes intake assessment, individualized care plans, home visit, basic counselling, care coordination, service linkage, and other clinical best practices relevant to promoting clients’ wellbeing.
2. Work with volunteers to provide befriending and support to clients.
3. Provide basic support services to caregivers to support their caregiving effort.
4. Develop caregiver support network within the specified service boundary.
5. Manage case documentation, home visit logs, and case statistics in timely and effective manner that complies with regulatory reporting standard.
6. Develop, review, and implement appropriate intervention strategies in consultation with supervisor.
7. Participate in case reviews, case supervisions, and case conferences.
8. Participate in the care network and coordination with stakeholders within centre’s service boundaries.
9. Assist in the outreach initiatives such as community mental health screening, health talk (when necessary), and promoting centre’s services to create public awareness of healthy aging.
10. Perform other duties assigned by the supervisor.

Description

You will be responsible for leading business and technology outcomes for assigned program, project, or contracted service.

You will leverage standard tools, methodologies, and processes to deliver, monitor, and control service level agreements.

Your day-to-day activities will revolve around managing the project team, ensuring timely delivery of project milestones, and ensuring that the project is delivered within budget.

You will be expected to be an SME in Program Project Management with expert proficiency.

Having expert proficiency in Business Process Design, M&A Integration Management Office (IMO), M&A Tech Integration Change & Comms, and Requirements Analysis will be advantageous.

Lead the project team to ensure timely delivery of project milestones.

Ensure that the project is delivered within budget.

Manage project risks and issues and develop mitigation strategies.

Provide regular project status updates to stakeholders and senior management

Requirements

Know-Your-Customer (KYC) and Anti-Money Laundering (AML) experience

KYC system experience - eg, Detica/ NetReveal system functionalities and/or technical understanding is a plus

System Implementation Project Management experience - ability to drive discussions, manage stakeholders and perform management updates in a timely and concise manner.

· Designing, installing, maintaining, and supporting the operation of the organization’s systems and networks.

· Responsible for managing SDLC and new systems implementations for streamlining operations within time and cost parameters as per organizations’ quality standards.

· Write technical procedures and documentation for the applications including production support procedure, user configuration guide.

· Provide technical expertise and solutions in integrating group solution with local systems and to support and enhance existing applications.

· Analyzing system requirements and ensuring that systems will be securely integrated with current applications.

· Defines the detailed object design and interaction, method level implementation details, and code structure needed to implement the technical design and product architecture.

· Review high level design to ensure the application is build-able.

· Creating detailed network and system architecture diagrams.

· Perform root cause analysis in defect resolution and while debugging.

· Providing technical direction and support during large-scale organizational projects.

· Identifies reuse opportunities and designs for reuse as appropriate.

· Monitoring system performance and troubleshooting issues.

· Conducting regular system audits and software upgrades.

· Leads and guides developers in day to day activities.

· Ensuring the constant availability of technical resources.

· Maintaining and improving the technical standards and procedures to ensure system integrity.

· Collaborating with other team members and departments to understand and develop system goals and functional requirements.

· Training and mentoring junior systems engineers and other team members.

· Documenting system processes and procedures and ensuring that documentation is kept up-to-date.

Responsibilities

• To be the liaison for the coordination of M&E works to ensure works are carried out as per schedule and local authorities’ quality standard.
• To supervise and ensure that work done are in accordance with the required drawings and specifications as per the contract.
• To monitor closely and report promptly work progress to superior.
• Coordinate and prepare technical submissions with sub-contractors and suppliers to ensure timely work arrangements.
• Manage project documentation and maintain accurate records.
• Ensure compliance with all health and safety regulations.

Qualifications

• Diploma in Civil Engineering/Building Construction/Mechanical Engineering or any equivalent qualification or relevant on-the-job experience are welcome.
• More than 3-years of work experience in the Construction sector would be preferred.
• Good team player and able to work independently.
