RESPONSIBILITIES

Job Description

• Receive and assess referrals to ensure appropriate placement in Nursing Home/Centre.
• Provide end to end casework management and psychosocial support services to residents, clients, and their families. Utilize appropriate frameworks for holistic assessment.
• Identify and develop individualized care plans together with clinical care team to meet the psychosocial and emotional needs of residents and clients.
• Address issues related to grief, adjustment, and coping with chronic illness or disability.
• Implement and facilitate appropriate casework interventions, including ongoing monitoring and evaluation of outcomes.
• Establish network with external partners and stakeholders for resourcing and ensure continuity of care of residents/clients across settings.
• Work closely with multidisciplinary team to ensure that holistic needs of the resident/client are actively considered in their care planning.
• Ensure "hands-on" approach to working with residents/clients through individual work, group work, counselling, or other appropriate modalities.
• Maintain accurate and current documentation of casework for assigned caseload.
• Work closely with residents, clients, and families to maintain or re-establish family relations when appropriate.
• Provide support in end-of-life care to residents, clients, and their families. This may include bereavement support to family members of the deceased and practical matters such as referral to casket company for last rite matters, after office hours.
• Conduct review of resident/client's socio-economic status and make appropriate recommendations for financial assistance if necessary.
• Participate in multidisciplinary meetings and work closely with clinical care team in discharge planning reviews.
• Attend case conferences, meetings, network with external agencies as required.
• Be a resource person for specific projects as needed.

• Bachelor Degree in Counselling/Social Work/Psychology or related discipline.

Requirements

• Preferably with at least 2 years' work experience in social services field (MOH or MSF) performing case management tasks.
• Experience working in a healthcare setting and knowledge of healthcare financing will be a plus.
• Able to work independently as well as be a team player.

Working Hours

Under normal circumstances the hours of duty will be as follows:

• 8.30am to 5.45pm (Monday to Friday, 45 minutes lunch break)

RESPONSIBILITIES

About the Job

To provide for the physiotherapy requirements of the Nursing Home residents in accordance with the Allied Health Professions Council guidelines and The Salvation Army's Mission and Philosophy of service provision.

Key Responsibilities

• Conduct initial assessments for residents new admission
• Conduct 6 monthly Care Plan and programme review
• Prescribe and execute individual and group therapy programme and activities for residents
• Able to assist and engage residents with behaivour of concern
• Review and revise programme schedule for therapy program with proper documentation
• Liaising closely with other healthcare professionals to encourage a holistic approach to Care
• Maintain an inventory list of therapy equipment and ensure regular maintenance is done
• Participate in multidisciplinary case discussions
• Participate in continuing education, training and research activities when opportunity arises

• Bachelor degree / Diploma in Physiotherapy
• Full registration with APHC

Preferred Candidate Attributes

• Minimum 1 years of working experience in ILTC sector

• Design creative artwork based on client's requirements/theme, knowledge of layout principles and aesthetics design concepts
• Conceptualise and create professional layouts or designs for promotional material, e-greetings and outdoor decoration projects (such as Christmas, CNY, Hari Raya etc.)

RESPONSIBILITIES

• As a member of the CREST (Community Outreach Team), participate in outreach events with the team to raise public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals through screening and assessment.

• Triage cases referred to centre and conduct initial assessments and intake interviews with clients to determine their needs and suitability for enrolment into the programme.

• Provide case management services including assessment, psychosocial interventions (including basic emotional support and counselling), & monitoring of clients and their caregivers enrolled into the CREST programme, via home visits and/or phone-calls.

• Coordinate care and support to clients with or who are at risk of mental health issues and/or dementia with the appropriate resources.

• Provide service linkages and referrals to right site persons requiring mental health/dementia-related support services to the appropriate services and resources.

• Advocate for clients' needs and assist in bridging gaps in service provision within the community

• Network and collaborate with government agencies (e.g., AIC), community partners (e.g., PA, Silver Generation Office, Family Service Centres, Active Ageing Centres), health and social care providers (GPs, hospitals, SSOs), to enhance support for persons with mental health conditions and dementia and their caregivers.

• Maintain accurate and up to date documentation of casework and maintain clinical/operational data for data submission and reporting, in accordance to programme requirements.

• Bachelor Degree in Counselling/Social Work/Psychology or related discipline.
