• Lifting heavy baskets stacked up to an estimated height of 1.8m/1.9m
• Working in tight spaces

Job Description:

• Provide counseling to patients and caregivers.
• Assess psychosocial and financial needs, and offer coordinated case management and interventions to help patients live well at home and in the community.
• Help patients understand their diagnosis and how it impacts daily activities.
• Support and motivate patients and caregivers, guiding them through challenges.
• Set realistic goals for patients and manage their expectations.
• Arrange temporary care or referrals for long-term care facilities when needed.
• Participate in ward rounds and case conferences, representing the patient’s social, emotional, and financial situation.
• Manage the volunteer program, including recruitment, screening, training, and retention.
• Undertake special projects and other tasks as assigned by management.

Requirements:

• Min. Degree in Social Worker or equivalent.
• Min. 4 years of social work, including experience in Nursing Homes.
• Registered Social Worker with Singapore Association of Social Work.
