About The Role
Community Outreach Team (CREST), serves as a community safety network for people with or at-risk of mental health conditions, including dementia, and their caregivers. CREST conducts outreach events to increase mental health awareness and targeted case finding to identify those with mental health needs.
CREST works with clients aged 18 years and above.
Reporting to the Group Supervisor, the Program Executive works closely with the team to serve clients and caregivers.
· Plan, implement and monitor the outreach, care process, referral and follow-up flow for the CREST service
· Document and evaluate programme progress
· Document and produce CREST service planning documents and manuals, including set up requirements and processes
· Conduct needs assessment to ascertain service needs and gaps for mental health in the community
· Collaborate with other agencies/organisations in planning and implementation of programmes
· Monitor trends and issues relating to mental health services and needs of clients and the community
· Outreach to corporates and community organisations to promote mental wellness, health awareness and information
· Engage partners to raise awareness, education and training on mental wellness via structured programmes
· Conduct screening within the community with agreed upon identification tools
· Provide the necessary care coordination and service linkages for identified clients from the initial referral
· Follow-up with clients for re-evaluation and provide support, as recommended in-care process flow
· Provide basic mental health awareness and information on managing the mental illness and coping strategies
· Provide basic emotional support to clients at-risk of mental health
· Compile and analyse collected data and submit reports as specified

Technical Competencies:

Preferred achievements / characteristics:
- Able to speak dialects will be an advantage
