Work Hours

Full-time employees must work a 5-day week, including a weekend. Your work schedule is pegged to the center's operation hours and operational needs. As a general rule, you will work the following hours:

• Weekdays: 12.30pm to 9.30pm (inclusive of one hour's meal break)
• Saturday or Sunday: 8.30am to 6.30pm (inclusive of one hour's meal break)
• 1 weekday off; 1 rest day

RESPONSIBILITIES

You will be part of the Community Case Management Service team, providing support to vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs. Key responsibilities:

• Provide intake and triage services for members of the community;
• Casework, counselling and service coordination for seniors and caregivers;
• Conduct psychosocial assessment and coordinate interventions with healthcare and community professionals to support seniors and caregivers;
• Document and review case, interventions and outcomes within set timelines, ensuring case documentation submitted on a timely manner
• Liaise and work with organisations, partners and volunteers to build an ecosystem of assets and resources to facilitate more effective work with seniors and caregivers.

Bachelor in Social Work or relevant fields (Psychology, Community Work); candidates from non-social work qualification will be considered as Social Work Executive.

Essential Duties and Responsibilities
