About the Role
As a Senior Staff Nurse (Home Care), you will play a vital role in delivering personalized and competent nursing care to patients in their homes. Reporting to the Nurse Manager (Home Care), you will provide holistic support to patients with advanced diseases, ensuring their well-being while guiding their families and caregivers. Your role will involve complex patient care management, direct nursing interventions, and multidisciplinary collaboration to uphold the highest standards of palliative care.
Key Responsibilities:
- Comprehensive Patient Care: Deliver safe, individualized, and competent nursing care to home care patients in accordance with best practices and statutory requirements.
- Complex Case Management: Manage patients with advanced conditions, demonstrating strong clinical skills, knowledge, and critical thinking in patient assessments and interventions.
- Care Coordination: Act as a liaison between families, caregivers, and physicians to ensure seamless communication and optimal care plans.
- Family and Caregiver Support: Educate, train, and provide emotional support to caregivers in managing patient needs at home, including equipment allocation and living adjustments.
- Treatment Administration: Perform direct nursing care, including medication administration, wound dressing, monitoring vital signs, and responding to patient needs.
- Monitoring and Documentation: Conduct regular assessments, maintain accurate documentation of patient conditions, and ensure compliance with care protocols.
- Multidisciplinary Collaboration: Participate in team meetings with doctors, social workers, and therapists to develop and implement effective patient care plans.
- Safety & Compliance: Adhere to infection control measures, medication storage protocols, and all regulatory nursing standards, ensuring patient safety at all times.
- Equipment & Resource Management: Monitor and manage medical supplies, ensuring cost-effective use of consumables while maintaining quality care.
- Public and Patient Engagement: Handle patient inquiries professionally and uphold Assisi Hospice’s high standards of service excellence.
Experience and Skills Required:
- Diploma or Degree in Nursing from a recognized institution, with preference on Oncology / Palliative Care / Gerontology / Geriartrics)
- Registered with the Singapore Nursing Board
- At least 2 years of post-registration experience, preferably in a hospital or hospice setting.
- Experience in palliative or home care will be an added advantage.
- Strong clinical and critical thinking skills in managing complex patient conditions.
- Compassionate, considerate, and passionate about providing holistic care.
- Excellent interpersonal and communication skills to engage with patients and families.
- Comfortable working in a dynamic healthcare environment.
- Able to converse in Chinese.
Working Hours
- Work Days: Mon - Fri (Must be comfortable to work on weekend/PH if scheduled to be on-call)
- On-call duties will be scheduled every 6-7 weeks
- Working Hours: 8am - 5.30pm
