The Manager is a specialist in a specific area of health services research (HSR) with good understanding of the broad HSR landscape and trend. As an Enrolled Nurse, you will play a critical role in ensuring the health and well-being of our clients:

• Provide nursing care and manage the medical needs of clients, including administering medication and supplements.
• Ensure clients’ safety and support their engagement in activities in a safe environment.
• Review daily reports on clients' activities and assess progress in terms of Activities of Daily Living (ADL) and Cognitive/Leisure Skills (CLS).
• Mentor and guide junior nursing staff to enhance team performance and client care.
• Assist staff nurses in managing patient care and conducting assessments.
• Coordinate clients’ medical appointments and follow-ups as required.

Other Responsibilities

• Participate in donor, volunteer, and fundraising events.
• Assist with any other duties as assigned by management.

Job Requirements

• Minimum of 2 years of related nursing experience.
• Professional Certificate / NITEC in Nursing and registered with the Singapore Nursing Board.
• Willing to do shift work.
• Experience working with persons with special needs or geriatrics is preferred.
• Ability to work both independently and collaboratively within a multi-disciplinary team.
• Strong communication skills and ability to work effectively with internal and external stakeholders.
• Hardworking, meticulous, and responsible.

Working Hours:

• 6 days a week with 2 shift hours.

Overview

Reporting to the Nurse Manager, Community Outreach Services, you are required to support the frail and home-bound PNC (Person Needing Care) with home personal care services / HPC Plus services.

Responsibilities:

• Conduct and perform clients’ assessment within 14 working days upon receipt of referral by using various tools provided by AIC including but not limited to the following: Modified Barthel Index (“MBI”) for rehabilitation status and Resident Assessment Form (“RAF”) to determine the patient’s current level of clinical care requirements.
• Review clients’ assessment on functionality and clients and/or caregivers’ satisfaction survey every 6-month period using various tools provided by AIC.
• Ensure cash collected from clients / receipts and account tallies.
• Ensure proper documentation processes are in place for audit purposes and good governance; Timely submission of reports to the governing authorities.
• Report and review feedback provided by care staff on clients’ progress especially Incident Report.
• Provide orientation, coaching and training to reporting staff on periodic basis.

Requirements:

• Registered Nurse, Certified by Singapore Nursing Board
• Degree / Diploma in Nursing
• Minimum 3 years of working experience in Geriatric Care/Hospital setting
• Passionate in working with elderly, willingness to learn
• Open attitude to work in eldercare
• Good understanding of working with older persons
• Passion and willingness to led a team
• Ability to speak one other local language is preferred (To communicate with non-English speaking seniors)
