We are seeking a highly experienced Patient Care Coordinator to join our team and contribute to our mission of exceptional healthcare delivery.
Key Responsibilities:
Help assist with the Doctor conducting comprehensive patient intake assessments, addressing physical, cognitive, psychosocial, cultural, and social needs in alignment with healthcare best practices.
Develop and implement tailored patient support plans in collaboration with the chiropractic team.
Oversee complex case management and coordinate multidisciplinary care for patients with diverse needs.
Help maintain accurate, detailed patient records in compliance with healthcare regulations and privacy standards.
Train and mentor new and existing team members, ensuring high standards of professional care and service delivery.
Serve as a liaison between the clinic, patients, and community healthcare providers to optimize patient care pathways.
Analyze and interpret patient feedback to develop quality improvement initiatives.
Qualifications and Experience Required:
Minimum 5 years of experience in aged care or a related healthcare role, including significant involvement in clinical assessment and case management.
Proven experience as a Clinical Nurse Specialist or equivalent role, with a focus on developing and implementing patient care plans.
Extensive knowledge of community healthcare facilities, aged care assessment guidelines, and government healthcare policies.
Demonstrated ability to lead teams, manage performance, and mentor staff in a healthcare setting.
Exceptional interpersonal, verbal, and written communication skills, with a proven track record of resolving conflicts and managing complex cases.
Advanced computer literacy, including experience with healthcare software (Cliniko or equivalent) and proficiency in Microsoft Office Suite.
Bachelor’s degree in healthcare related field or equivalent; postgraduate qualifications in healthcare or related fields preferred.

Demonstrated experience in quality improvement initiatives and change management.
Additional Requirements:
Candidates must have managed aged care assessments and multidisciplinary teams in their current or recent roles.
Experience in a managerial or acting managerial position will be highly regarded.
Extensive knowledge of health roster systems, data collection tools, and professional guidelines in healthcare is essential.
