• Counsel and support the patients and families associated with the illness.
• Work closely with community partners for the welfare of the patients.
• Deliver public education talks.
• Maintain working relationship with other disciplinary teams on patient care plans.
• Work with other provider to provide appropriate services to ensure continuity of care.
• Provide training and guidance to juniors and new hires
• Share the sessions with stakeholders and other professionals.
• Maintain the right documentation for case management and audit purposes.

Requirements:

• Possess a minimum Degree in Social Work/ Post graduate diploma in social work or equivalent
