Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. Description:

Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.

Admissions

• Provide financial counseling upon admission

Case Conference

• Provide counseling to patients with psychosocial issues
