Requirement:

• Bachelor’s Degree in Social Science (Social Work), Degree or Graduate Diploma in Social Work is a requirement.
• Certification in “Introduction to Family Violence”; “Management of Family Violence” and cognitive behavioural therapy, family-centered social work practice will be an asset, although not essential.

STATEMENT OF PURPOSE

This role support and assist the nursing team and the medical social work team in the administrative tasks, information gathering, form filling, fieldwork and other ad hoc duties.

KEY RESPONSIBILITIES

1. Pre-admission/Admission Support

1.1. Orientate new residents and their NOKs to the nursing home.

1.2. Ensure residents settle well in the nursing home.

2. Casework (non-complex case)

2.1. Engagement and Assessment

• Establish rapport and build professional relationship with resident and significant others.

• Identify and clarify the key issues to the problem situation and gather relevant information from resident and NOKs.

• Conduct bio-psycho-social-spiritual assessment to assess and identify the severity and key risks and protective factors for intervention.

• Engage residents, families, multi-disciplinary team and relevant stakeholders to gather holistic and relevant information.

2.2 Goal Setting and Intervention Plans

• Analyse and evaluate different courses of action, when appropriate with multi-disciplinary team and relevant stakeholders.

• Formulate appropriate strategies for action.

• Conduct care planning and goal setting with residents based on standard guidelines and professional assessments.

• Set clear contracts and goals with residents - Engage residents to sustain effort in problem-solving.

• Apply appropriate assistance schemes.

• Collaborate and negotiate with stakeholders to mobilise community resources.

2.3 Case Review and Documentation

• Follow up and review care plans and provide recommendations to modify care plans where necessary.

• Evaluate the process as well as the outcome objectively and recommend future improvement.

• Provide documentation and timely updates of assessments and intervention to be used by multi-disciplinary profession.

• Work with MSWs on providing social reports with proper consent to relevant stakeholders to ensure continuity of care.

3. Keep residents meaningfully engaged

3.1. Understand interests of individual residents.

3.2. Organise activities that are of interest to residents.

3.3. Engage and motivate residents to participate in the activities.

3.4. Work with volunteers and Volunteer Manager in activities planning and implementation.

3.5. Review and evaluate the effectiveness of activities in improving the well-being of residents or handle the situation.

4. Support volunteer program

4.1. Provide feedback on possible new volunteering opportunities based on the needs observed on the ground.

4.2. Work closely with the Volunteer Manager on effective mobilising of resources to support volunteer program.

4.3. Activities Programme

• Provide supervision during implementation of activities.

• Provide feedback to volunteers on areas of improvement via Corporate Communication in charge.

5. Support Advance Care Planning (ACP) program

• Initiate ACP awareness with residents and their NOKs.

• Facilitate General ACP or PPC with residents and their NOKs for those who are ready.

• Review ACP plans regularly

• Assist residents in fulfilling their care preferences and last wishes.

1. The staff will undertake projects relevant to the post, as requested by HOD or ED.

A) KEY RESPONSIBILITIES AND ACCOUNTABILITIES

The Social Worker works in the field of Social Work. He/She is qualified and trained to provide interventions, perform community work and update the professional practice in Social Work. He/She will conduct casework, groupwork as well as community development programmes to address service gaps. He/She will need to provide supervision to junior Social Worker(s) and/or Social Work Associate.

1. Case Work Management

a) Conduct needs and intake assessments of client/family and provided social report with recommendations, to allow them to live independently and be participative in the community.

b) Conduct case work management for cases of different risk levels.

c) Coordinating service referrals and access to community resource.

d) Able to provide psycho-emotional support for clients and caregivers.temporary

e) Build partnerships and collaborate with other agencies to tap on community resources to provide intervention support to clients and empower/enable clients e.g. for the smooth transition of clients back to community, vocational pursuit and/or independent community living etc.

f) Participate and contribute professionally as a member of a multi-disciplinary team in SPD. Work with therapists and related staff for client’s needs and involve in case discussions.

g) Document sessions/interventions accurately and consistently, and to maintain case files and prepare reports according to the required documentation standard with minimal supervision.

h) Conduct closure of cases upon achieving sustainable service outcomes for clients with minimal supervision.

2. Community Work and Outreach

a) Identify the changing trends and needs of clients.

• Possess Degree in Social Work or Degree with a Graduate Diploma in Social Work from an accredited institution.

• Lead psycho-education groups that aim to provide information to and/or educate groups of target audience.
• Facilitate high risk therapeutic group work under supervision.
• Review group work interventions for improvements.

Perform community development and outreach work

• Conduct environmental scans and community need assessments, monitor trends and issues relating to needs of clients and community.
• Design, develop and implement community-specific development programmes in meeting identified community needs.
• Participate in community development initiatives and provide recommendations for consideration.
• Review community development initiatives for improvements and address service gaps.
• Collaborate and network with other agencies to pool community resources.

Professional Development & Education

• Engage in learning opportunities (e.g. participate in community of practice meetings to develop skills and capabilities).
• Attend professional development courses to acquire professional knowledge and skills.
• Supervise junior social workers, interns and / or students on placement.

Programme Development & Implementation

• Participate/ implement programmes (e.g. strengthening activities appropriate to clients’ profile)
• Review and refine programmes to target client groups or other stakeholders/ partners/ agencies
• Identify service gaps in response to client needs and make recommendations to improve service provision of programmes.

Uplift professional practice in social work

• Participate in research to uplift the professional practice in social work.
• Support the implementation of standards of care, organizational health policies, procedures and programmes.
• Identify areas of improvement in models of care and service delivery based on evidence-based practices.
• Respond to crisis situations based on organizational standard operating procedures.
• Assist to develop, maintain & review the casework & counseling system
• Engage in multi-disciplinary team research projects.

• Perform other duties as assigned by the Assistant Director & Head of FSC/Director/Management.

REQUIRED QUALIFICATIONS AND EXPERIENCE

• Minimum Bachelor degree in Social Work or Graduate Diploma in Social Work or other relevant Allied Health discipline preferably accredited with Singapore Association of Social Workers (SASW).
• Min 3-5 years of experience in social service sector.
• Knowledge of current social, economic, and governmental conditions and issues.
• Knowledge of human behavior and able to work effectively with individuals and groups.
• Familiar with at least one theoretical framework of social work intervention.
• Good written, listening and verbal communication and interpersonal skills.
• Ability to establish effective working relationship with staff, other agencies and the community.
