Job Responsibilities:

About the Role:

Interview and assess potential and existing clients:

• to formulate personalised service and care plans,
• to prepare case reports,
• to provide information & referral to external relevant agencies for appropriate services.
• to provide and coordinate support services for aLife’s clients.
• to monitor, follow up and review care/case plans and provide recommendations to modify care/case plans where necessary.
• to liaise with clients, clients’ families, volunteers and external agencies.
• to assist in the planning, implementation and evaluation of programmes, events and activities.
• to upkeep clients’ documents, case/counselling notes and individual case files.

Experience & Qualifications: Attributes & Qualifications:

• Positive, highly-motivated, self-starter & passionate about helping people
• Able to work on Saturdays (full-day) as and when required.
• Social work degree with at least 3 years of experience in casework.
• Experience in counselling, especially to women with unsupported pregnancies.
