Job Description

· Co-ordinates social services through a case management approach.

· Addresses psychosocial needs through care and counselling.

· Assists staff members in understanding the significant social and emotional factors related to the resident's health problems. Liaises with internal colleagues on matters / feedback relating to residents, fellow colleagues

· Participates in the development of the plan of care. Attends multi-disciplinary meetings with medical staff, nurses, other healthcare professional and non-professionalsto discuss, propose and coordinate care plans for residents.

· Works with other health professionals significantly involved in the care of the resident.

· Discusses long-term care issues with the resident and family.

· As an case admission coordinator - liaises and works with all referral sources; plan and coordinate case admission

· Screens the social background of all new applicants and applies means test to determine chargeable fees;

· Advises residents and family members on chargeable fees and refers them for subsidies, financial aid if necessary.

· Performs field work involving visiting residents at wards and other social agencies to help residents alleviate or resolve their medico-social problems

· Prepares social and other related reports for file, management and other related institutions / agencies

· Undertakes to keep herself / himself well informed on government guidelines on healthcare and social services available within community.

· Maintains case records on resident manual / computerised. The records kept include individuals who have been rejected by doctors for admission and enquiries of public on MSW services.

· Assist to conduct household means testing for residents and application for MOH subsidy funding and other source of financial assistant

· Assist in residents’ Medifund / Medifund Silver applications.

· Advises defaulters in formulating repayment plans when requested by the finance department and other healthcare institutions or services.

· Involved in resident case management

RESPONSIBILITIES

Casework and Counselling

• Engage client and significant others (e.g. family, caregivers, etc.) in a therapeutic manner to understand their needs
• Manage and overcome reluctance and resistance to change
• Implement and follow up on care/ case plans and interventions to determine the suitability of care/ case plans in addressing client's needs and modify them where necessary
• Draft social reports for complex cases to be reviewed by supervisor
• Conceptualise and identify appropriate approaches to develop care/ case plan for
• client
• Help clients to develop and choose strategies and formulate viable plans
• Represent case in case conference

Group Work

• Target and reach out to individuals and/ or targeted groups who may benefit from group work conducted by organisations/ agencies
• Plan, implement, monitor, evaluate and modify therapeutic group work to address issues of target groups
• Facilitate high risk therapeutic group work under supervision

Community Work

• Conduct environmental scans and assess community needs
• Design, develop and implement community-specific development programmes (e.g. aging community in a specific neighbourhood)
• Participate in community development initiatives and provide recommendations for consideration

Professional Development & Education

• Engage in learning opportunities (e.g. participate in community of practice meetings to develop skills and capabilities)
• Attend professional development courses to acquire professional knowledge and skills
• Supervise interns and/ or students on placement
• Guide fresh/ junior social workers

Research

• Participate and support research
• Engage in multi-disciplinary team research projects Programme Development & Implementation
• Participate/ implement programmes (e.g. strengthening activities appropriate to clients' profile)
• Review and refine programmes to target client groups or other stakeholders/ partners/ agencies
• Identify service gaps in response to client needs and make recommendations to improve service provision of programmes

• Degree in Social Work/ Post Graduate in Social Work

• At least six years' experience working in the social service sector, (Family Service Centre) preferably with clients with disability, neurocognitive and/or mental health issues
• At least two years' supervisory experience in the social service sector
• Good teamwork and interpersonal skills with the ability to work in a multidisciplinary team; and a high level of integrity
