RESPONSIBILITIES

• End-to-end casework of clients from information gathering, assessment, case management, working with systems, case conferences to case closure.

As a member of Gracehaven Social Services Team, you are expected to provide effective Case Work Services for children, youth and their families. To partner with the different professionals to deliver high quality of care to meet the diverse needs of children and young persons entering the residential service. To ensure that the emotional, social, behavioural, developmental, therapeutic and educational needs of clients are met during the client's stay in the Home.
