We are seeking a Social Worker to join The Helping Hand team. The role involves:

• Providing social work services, including casework management, counseling, home/site visits, and interview assessments.
• Formulating, implementing, and evaluating programs that cater to our beneficiaries’ needs.
• Working with external stakeholders and other teams to ensure efficient delivery of our programs / services to our beneficiaries.
• Maintaining accurate and timely updates of case recordings, client profiles and other related casework management processes.
• Maintaining and compiling statistics for reporting, funding, or audit purposes.
• Other duties as required by the Social Work Manager.

Requirements

• Recognised Degree / Postgraduate Diploma in Social Work/Counselling or related field.
• Professionals that are accredited as a social worker / social service practitioner with SASW are preferred.
• Prior experience in family intervention would be an advantage.
• Minimum 3 years of relevant working experience.
• Good teamwork and interpersonal skills with the ability to work in a multidisciplinary team.

• Responsible for supporting Medical Social Workers (MSW) team in direct patient work, administrative work and general duties
• Manage routine cases assigned by MSWs independently
• Provide training for new social work and administrative assistants
• Conduct financial assessment on eligibility of applicants for the various financial schemes
• Conduct field visits and facilitate discharges by accompanying patients home or to social service organizations
• Able to independently take on inpatient and outpatient casework, doing follow up work for discharged patients and networking with community services to facilitate seamless care

Job Requirements:

• Min Diploma with social work / conselling / Psychology, etc

Mon-Fri 830am-6pm, 1 half day sat once a month
