RESPONSIBILITIES

1. Receive and assess referrals in conjunction with clinical team to determine suitability and ensure appropriate placement in Home/Centre. Make recommendations to ED or designate for approval of admission and funding.
2. Provide case management and psychosocial support services to residents, clients, and their families. Utilize appropriate frameworks for holistic assessment (e.g., BPSS model, Systems theory).
3. Identify and develop individualized care plans together with clinical care team to meet the psychosocial and emotional needs of residents and clients.
4. Address issues related to grief, adjustment, and coping with chronic illness or disability.
5. Implement and facilitate appropriate casework interventions, including ongoing monitoring and evaluation of outcomes.
6. Establish network with external partners and stakeholders for resourcing and ensure continuity of care of residents/clients across settings.
7. Work closely with multidisciplinary team to ensure that holistic needs of the resident/client are actively considered in their care planning.
