Working Hrs: Monday to Friday, 8.15am to 5.30pm

Job Description
· Co-ordinates and involved in residents case management through social services approach.
· Addresses psychosocial needs through care and counselling.
· Assists staff members in understanding the significant social and emotional factors related to the residents’ health problems. Liaises with internal colleagues on matters / feedback relating to residents, fellow colleagues
· Participates in the development of the plan of care. Attends multi-disciplinary meetings with medical staff, nurses, other healthcare professional and non-professionalsto discuss, propose and coordinate care plans for residents.
· Works with other health professionals significantly involved in the care of the resident.
· Discusses long-term care issues with the resident and family and conduct ACP/PPC/EOC.
· As a case admission coordinator to liaises and work with all referral sources, screen social background, plan and coordinate case admission.
· Advises residents and family members on chargeable fees and refers them for subsidies, financial aid if necessary.
· Performs field work involving visiting residents at wards and other social agencies to help residents alleviate or resolve their medico-social problems
· Prepares social and other related reports for file, management and other related institutions / agencies
· Undertakes to keep herself / himself well informed on government guidelines on healthcare and social services available within community.
· Maintains case records on resident manual / computerised. The records kept include individuals who have been rejected by doctors for admission and enquiries of public on MSW services.
· Assist to conduct household means testing for residents and application for MOH subsidy funding and other source of financial assistant eg MFEC and other government scheme
· Assist in residents’ application for Medifund / Medifund Silver / FAS.
· Assist in coordinating government payout, GovCash listing and obtaining consent.
· Advises defaulters in formulating repayment plans when requested by the finance department and other healthcare institutions or services.
Qualifications
Degree in Social work with clinical experience in a setting dedicated to the care of the older person or care management and preferably, post basic qualifications in gerontology.
Experiences
Equipped with experience in healthcare or long-term care setting, preferably working with elderly.
Strong assessment, counselling and communicate skills.
Knowledge of relevant regulations
