RESPONSIBILITIES

You will be part of the Community Case Management Service (CCMS) team, providing support to vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs.

Key responsibilities:

• Conduct intake and triage services for members of the community
• Provide casework, counselling and service coordination for seniors and caregivers
• Conduct psychosocial assessment and coordinate interventions with healthcare and community professionals to support seniors and caregivers
• Document and review case, interventions and outcomes within set timelines

Description:

Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. Admissions

Case Conference
