• Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.

Admissions

Case Conference

• Review social aspects inward with regards to activity participation, visitors and befriending, financial consideration status, etc.

• Review supporting documents for referral to other care options and seek financial assistance for home equipping as recommended by the therapist where required
• Provide counseling to patients with psychosocial issues
• Facilitate family conferences for discussion on financial considerations, if necessary
• Work with the Nursing team to review care for residents and understand their social situation

Help create and maintain a child-centred, homely therapeutic community that is responsive to the developmental needs of residents, where they are enabled to grow holistically, work towards emotional stability and develop their full potential.

Duties (not limited to the following) include:

• Carry out the daily operational duties such as supervising the residents' daily routines, including ensuring they have their meals, going to school, doing chores, attending programmes, keeping documentation up to date, maintaining the cleanliness of the premises, etc.

RESPONSIBILITIES

• This role will support the Manager to oversee the quality of practice and professional practice in the CCKS Team

• Supervise the programme intervention, provide consultations on design and delivery of the programme to ensure that the delivery of services matches government and community priorities for the social service sector.

• Provide practice supervision to senior staff/ peers/ and external parties and institutions, develop supporting mechanisms to support capability development and overall service quality adopting best management principles and practices.

• Possess and impart in-depth knowledge in the area of practice in which the professionals operate, drive practice governance within team, service group/organisation and training needs analysis at the centre/service group level.

• Provide internal training to practitioner and conduct external training that will enhance the competency of the profession within the sector.
