Job Description

• To provide nursing care to the daycare clients. Tasks can include: prepare client’s assessment and clinical documentation, medicine packing, wound care, centre-based nursing, updating caregivers on client’s condition and carrying out competency audits on healthcare assistants and community care associates.
• Monitor outcomes of services and perform escalation of cases if there is a need for more acute management of clients;

The Community Nurse (Home Care) is responsible for planning; coordinating, directing and evaluating the patient care services and provides supervision serving as the vital link between the vision of CWA and a team of Community Caregivers (CCGs). The College Nurse will oversee the provision of a clinically effective and high quality nursing care and welfare service to students, and First Aid care to all members of the school community as required. The College Nurse will also oversee the provision of a holistic approach to the care of pupils, including health promotion, PSHE, and the assessment and treatment of illness and injury.

Duties and Responsibilities:

• Provide triage and nursing care in one of the three College health centres on a walk-in basis, dealing with medical, physical and psychological needs of the students and wider college community;

Role & Responsibilities

Reporting to the Centre Manager of the newly opened REACH Senior Care Centre in Senja, the Care Staff will be responsible for the following key areas:

• Provide personal care assistance to the elders (clients) and ensure they receive the best possible care during their time in REACH Senior Care Centre.
• Assist the elders to take part in daily activities including serving meals to them and assisting them with toileting.
• Plan and conduct person-centered programmes to meet elderlies' cognitive, social, and physical needs
• Maintain up-to-date documentation of clients' conditions and care treatments and provides inputs for the development and review of care plans.
• Ensure an efficient operation of REACH Senior Care Centre, along with keeping it as clean and tidy as possible.
• Responsible to ensure house rules are followed by clients and recommend measures to mitigate risk to the elder.

Requirements

• Minimum Secondary School certification
• Caring, patient, and willing in serving elderly clients
• Prior experience with caregiving duties in Hospitals, Nursing Homes or with individual’s parents would be advantageous
• Team player with great interpersonal communication

Job Description

1. Assesses residents within 24 hours of admission.

2. Develops a nursing care plan and carry out nursing care based on care plan.

3. Assists in organising and coordinating activities directed at enhancing the quality of life of residents.

4. Accountable for the residents' personal hygiene, grooming and their activities of daily living.

5. Identifies and initiates appropriate preventive and rehabilitative nursing procedures.

6. Prepares and maintains proper and accurate clinical documentation.

7. Ensures safe custody and keep proper records for Controlled Drugs and Poisons.

8. Sees to the proper administration of prescribed medications and treatment.

9. Makes professional judgement and decisions in rendering nursing care to the residents according to individual's needs.

10. Supervises the enrolled nurse, nursing aides and healthcare assistants in the delivery of nursing care based on the care plan so as to maintain adequate standard of nursing care.

11. Guides, educates and trains new staff.

12. Co-ordinates with other staff members and update them on changes in the residents' conditions and needs.

13. Co-ordinates with other departments such as housekeeping and maintenance to ensure a safe, clean and properly maintained environment.

14. Evaluates the resident's response to care interventions on a regular basis and recommends revision of care plans as necessary.

15. Keeps abreast with the latest developments in the care of the elderly by reading nursing literature, attending lectures and talks.

16. Do shift duty and be on-call whenever necessary.

17. Any other duties as assigned by Supervisor.

18. Will oversee the ward matters and to train and guide Staff Nurse.
