Responsibilities

a. Comply with approved standards of nursing practices

b. Assist the Staff Nurse to supervise care staff and healthcare attendants

c. Work with Staff Nurse to uphold good outcomes through quality care

d. Assist the Staff Nurse in assessment of new admission of residents

e. Provide basic nursing care to the residents

f. Carry out nursing rounds with the Staff Nurse

g. Supervise care staff to ensure comfort and good personal hygiene of the resident under her/his care, e.g. bed bath/shower bath, changing of clothes and bed sheets and tidiness of the room

h. Supervise care staff to check, monitor and record residents’ vital signs such as temperature, pulse, respirations, blood pressure and apex heart beats

i. Assist the Staff Nurse to ensure residents take their medicines

j. Guide care staff to assist in the transfer and positioning of non-ambulant residents

k. Supervise care staff to assist in serving diets and feeding residents who are unable to feed themselves

l. Maintain accurate documentation of care given

m. Assist in the admission and orientation of new residents

n. Perform rehabilitation procedures such as motion exercises for the residents and walking them

o. Organise and send residents for appropriate therapy as prescribed, e.g. occupational therapy, laundry, kitchen and ward work and garden cleaning

p. Coordinate visits for medical check-ups, arranging transport and accompanying residents to the hospital/polyclinic for follow up appointments

q. Initiate activities as first responder at emergency situations, alerts and supports the RN in such situations

r. Work with the Staff Nurse to maintain a safe and clean environment in the nursing home

s. Carry out any other duties as assigned by the Supervisor/Staff Nurse

REQUIREMENTS

• National ITE Certificate in Nursing or its equivalent recognized by Singapore Nursing Board
• At least 2 years’ experience as Enrolled Nurse

PROFESSIONAL LICENCE

• Enrolled with Singapore Nursing Board (SNB)
• Basic Cardiac Life Support (BCLS) certified

Job Scope:

• Clinic Nursing duties in ENT / Gastro / Healthscreening clinic

Office hours (5.5days)

Key Summary

The Assistant Nurse Nurse Clinician is responsible to assist Director of Nursing and her team to monitor and evaluating the effectiveness, safety and efficiency of community nursing practices within the Foundation’s range of clinical services.

Key Responsibilities

• Assist to develop and implement evidence-based clinical practices, guidelines, policies and procedures, including defining nursing competencies and care standards through working with teams
• Assist to collaborate with cross-functional teams in clinical service strategic planning work and identify areas of opportunities and growth for clinical programs and service
• Assist to network and conduct market research and landscape studies to stay abreast of emerging trends / technologies / innovations to support the development of new nursing care models and implements strategies to augment care delivery with seamless integration of programs and services among clinical teams within the Foundation. Cross-functional teamwork and inter-agency collaborations are paramount for success
• Experience in community and gerontological nursing practices, coupled with a good understanding of the health profile and population needs of older people in the community, to ensure service alignment with the Foundation’s philosophy of care
• By applying evidence-based community gerontological nursing practices, he/she shall effectively manage clients with complex care needs through direct care
• Act as a floater to work with and assist the team across the Foundation and evaluate care standards and quality, assess DON/ NC on nursing competency, including identification and implementation of training programs to support the on-going professional development and retention of nursing staff.
• With collaboration with cross-functional teams, to develop and implement audit protocols and procedures to evaluate compliance, identify and address gaps in clinical care and service delivery as well as be involved in risk management and clinical excellence initiatives
• Assist in quality improvement initiatives and conduct/support clinical research projects and advocacy works to advance knowledge and improve clinical outcomes
• Participate in collaborative projects with internal and external stakeholders to advocate for quality of care to community dwelling older adults

Key Requirements

• Minimum Masters/Bachelor in Nursing with a Post-Graduate/Specialist Diploma in Gerontological Nursing/Gerontology
• Additional professional qualification/certification in public health, public policy, business management or equivalent will be advantageous
• Registered with the Singapore Nursing Board
• At least 8 – 15 years of nursing experience, of which 4 - 6 years are in clinical supervision, nursing training and education
• Hands-on experience with older adults in community-based care settings is essential
• Prior appointment in management roles and work experience in case management or care coordination will be advantageous
• Extensive clinical knowledge and skill
• Competent communication, leadership and negotiation skills
• Resilient and resourceful with a creative flair and aptitude to excel in a dynamic environment with evolving requirements and different team dynamics.
