Position Overview:

We are seeking a compassionate and dedicated Case Worker to join our team. As a Case Worker, you will play a pivotal role in helping individuals experiencing homelessness or housing instability secure stable housing, access essential services, and work towards greater independence. This is an opportunity to make a meaningful difference in the lives of those we serve.


Key Responsibilities:


- Assess the compatibility of individuals to be paired for shared housing and manage the pairing process effectively.

- Act as an advocate for clients in interactions with social service agencies, and other organizations.

- Handle and resolve disputes that may arise among clients sharing housing arrangements.

- Maintain a thorough knowledge of local community resources and provide referrals and matching services to clients based on their needs.

- Conduct comprehensive assessments of clients' needs and goals.

- Develop plans and goals in collaboration with clients.

- Connect clients with social services, financial assistance, employment assistance, and other community resources.

- Provide ongoing support and advocacy for clients as they work towards achieving their objectives.

- Facilitate sessions on topics such as financial literacy, job readiness, and life skills.

- Maintain accurate and up-to-date client records.

- Collaborate with a team of professionals to ensure comprehensive care and support for clients.

- Monitor progress and outcomes, and make adjustments to case plans as needed.
