Working hours: 44hours / week (Rotating Shift)
AM 8.30am to 6.30pm
Day Duty 8am to 8pm (once a month)
Required to work weekends and public holiday
Salary : $3,500 - $5,000 (depend on exp)
Location: Buangkok
Job Responsibilities
- Assess needs of seniors with care/service needs and formulate care plans for seniors with care/service needs
- Assist in the development and implementation of programmes for seniors through data collection and the conduct of basic analysis
- Coordinate care for the seniors with relevant stakeholders
- Documentation of case notes
- Facilitate timely and appropriate service referral and provision to seniors
- Participate in case discussions with external service providers
- Provide case management services
- Perform needs assessment of seniors living in the community
- Prepare data for submission to relevant authorities
Requirements
- Diploma/Degree in Social Work
- Experience in Social Service Sector or related field
- Proficient in MS Word, Excel and PowerPoint.
