Job Description:
- Able to assist patient to be transferred to step-down care facility and mobilise required community resources to ensure patient’s needs are met upon return to the community close collaborations with internal and external resources to ensure smooth transition of patient to the community
- Familiar with the step-down care referral system and MOH guidelines.
- Provide psycho-social assessment to determine patient’s needs
- Able to communicate effectively with patient to help them understand their eye condition and make informed decision on their treatment plan
- Have case conference with the medical, allied health and nursing and finance dept to ensure patient will receive adequate and timely treatment.
- Ensure compliance with MOH criteria for financial assessment.
- Ensure proper documentation according to MOH requirements for annual audit
- Undertaking continuous professional - related courses/conferences/attachments with local/overseas attachments
- Follow the national guidelines on management of vulnerable patients.
- Able to assess and identify stressors/symptoms of vulnerable patients and to activate resources timely to prevent grievous hurt.
- Proper documentation of case
- Submission of statistics to required ministries
- Perform any ad hoc duties as and when assigned by Master MSW I, II or HOD, Medical Social Services/Quality Service Director and above
Job Requirements :
- Possessed at least a Degree in Social Work
- Possessed at least 2 years of expericne in Medical Social Work Services
- Have relevant knowledge in eye diseases to provide counselling to patient on their condition and the necessary treatment.
